Some of these savings are outweighed by a relatively long learning curve for physicians, which has likely slowed the adoption of the procedure in the U.S. Although transradial procedures often have less complications than femoral procedures, they have some shortcomings. Non-invasive BCIs have also been applied to enable brain-control of prosthetic upper and lower extremity devices in people with paralysis. For example, Gert Pfurtscheller of Graz University of Technology and colleagues demonstrated a BCI-controlled functional electrical stimulation system to restore upper extremity movements in a person with tetraplegia due to spinal cord injury. Between 2012 and 2013, researchers at the University of California, Irvine demonstrated for the first time that it is possible to use BCI technology to restore brain-controlled walking after spinal cord injury. In their spinal cord injury research study, a person with paraplegia was able to operate a BCI-robotic gait orthosis to regain basic brain-controlled ambulation.

A physiotherapy program will improve joint range of motion of the paretic limb using passive range of motion exercises. When increases in activity are tolerated, and stability improvements are made, patients will progress from rolling to side-lying, to standing (with progressions to prone, quadruped, bridging, long-sitting and kneeling for example) and learn to transfer safely (from their bed to a chair or from a wheel chair to a car for example). Assistance and ambulation aids are used as required as the patient begins walking and lessened as function increases. Furthermore, splints and braces can be used to support limbs and joints to prevent or treat complications such as contractures and spasticity.

Many types of balance disorders will require balance training, prescribed by an occupational therapist or physiotherapist. The Shriner's Burns hospital on the Campus of the University of Texas Medical Branch in Galveston, Texas Treatment areas cover a wide range of pediatric orthopaedics, including scoliosis, limb discrepancies, clubfoot, hip dysplasia, and juvenile idiopathic arthritis, as well as cerebral palsy, spina bifida (myelomeningocele), and other neurological conditions that affect ambulation and movement. The major focus of Weiner’s career has been the study of MS, which he began in 1972 as a resident in neurology at the Peter Bent Brigham Hospital in Boston. He has published over 200 articles on all aspects of MS including basic studies, animal models and clinical trials. In 1983, he alongside Stephen L. Hauser, now Director, UCSF Weill Institute for Neurosciences and Professor of Neurology at University of California, San Francisco, published a seminal article on the treatment of MS in The New England Journal of Medicine. This demonstrated in a controlled trial a profound effect of the immunosuppressant and chemotherapy drug cyclophosphamide in stopping active, progressive MS. The article introduced the timed 25-foot walk (ambulation index) that subsequently has become a classic clinical measure in MS. In 2000, Weiner founded the Partners Multiple Sclerosis Center at Brigham and Women’s Hospital. In general, however, lower-extremity spasticity in spastic diplegia is rarely so great as to totally prevent ambulation—most people with the condition can walk, and can do so with at least a basic amount of overall stability. Regardless, from case to case, steeply varying degrees of imbalance, potential tripping over uneven terrain while walking, or needing to hold on to various surfaces or walls in certain circumstances to keep upright, are typically ever-present potential issues and are much more common occurrences amongst those with spastic diplegia than among those with a normal or near-normal gait pattern. Among some of the people with spastic diplegia who choose to be ambulatory on either an exclusive or predominant basis, one of the seemingly common lifestyle choices is for the person to ambulate within his or her home without an assistive device, and then to use the assistive device, if any, once outdoors. Others may use no assistive device in any indoor situation at all, while always using one when outdoors.

BioMarin announced that the price per infusion is $27,000, coming to $702,000 per year for treatment, though using Medicaid can decrease the cost. In March 2018, cerliponase alfa was approved in the United States as a treatment for a specific form of Batten disease. Cerliponase alfa is the first FDA-approved treatment to slow loss of walking ability (ambulation) in symptomatic pediatric patients three years of age and older with late infantile neuronal ceroid lipofuscinosis type 2 (CLN2), also known as tripeptidyl peptidase-1 (TPP1) deficiency.
