The Mid- Suburban League came into existence for the 1963–64 school year. The MSL and Des Plaines Valley League both broke off from an interim suburban league that dated back several decades. The original members of the MSL were Deerfield High School, Forest View High School, Glenbrook North High School, Glenbrook South High School, Maine West High School, and Prospect High School. Not even a year old, the conference announced expansion for the 1965–66 season with, Wheeling High School, Niles North High School, and New Trier West High School joining as the schools opened.

In fall 1978, Washington High School merged with Monroe High School and became Washington-Monroe High School. Monroe H.S. was an all-girls vocational sister school to Benson Polytechnic High School. After the merge, the old Monroe High School building housed a number of programs until 1994, when it became da Vinci Arts Middle School. It was established in 1917 at Southwest 14th and Morrison and was named Girls Polytechnic High School until fall 1967, when it was renamed James Monroe High School. Monroe High School had only 470 students in fall 1977, the smallest enrollment of any public high school in Portland.
The conference originally had four members: Central, Cleveland, Humboldt and Mechanic Arts. In 1911, Cleveland High School changed its name to Johnson High School, and in the 1920s Washington High School and Harding High School joined the conference. In 1941 the smaller high schools of Marshall High School, Monroe High School, Murray High School and Wilson High School were added to the conference, bringing the number of schools in the conference to ten. In 1953, Marshall closed as a senior high and ten years later, Wilson would also close, with most of its students attending the newly builtHighland Park High School.

Triton Senior High was established in 1990 when the communities of Dodge Center, West Concord and Claremont consolidated to create one high school and junior high school. The current Fairborn High School was built as Park Hills High School in the late 1960s. Fairborn Baker High School (now Baker Middle School) and Park Hills High School merged into Fairborn High School in 1982. It has its students from Arlington Heights and also enrolls students from Prospect Heights, Mount Prospect and a small fraction of Des Plaines. Named after writer John Hersey, it is part of Township High School District 214 which also includes Buffalo Grove High School, Elk Grove High School, Prospect High School, Rolling Meadows High School, and Wheeling High School.
The James River District was formed in 1999 with seven charter members, Central High School (also known as Lunenburg or Lunenburg Central), Prince Edward County High School, Buckingham High School, Amelia County High School, Randolph-Henry High School, Goochland High School, and Cumberland High School. Every school except for Goochland is located in the Southside Virginia region. After the 1999 season, Prince Edward moved to a different district. Membership remained the same until the 2009 season when Goochland High School moved to a different conference, which left only Amelia, Buckingham, Randolph-Henry, Cumberland, and Lunenburg.
Los Altos High School was established in 1954 on the site of an orchard. Initially, Los Altos High School served the mostly white, middle to upper class populations of Los Altos and Los Altos Hills. During 1981 redistricting efforts, the district closed its oldest high school, Mountain View High School, which was located on Castro Street in downtown Mountain View. Awalt High School was then renamed Mountain View High School, while Los Altos High School replaced its Knight mascot with the Eagle mascot from the old Mountain View High School.

Ottumwa High School and Keokuk High School, as the only remaining members, formed the Southeast 7 to meet their needs for a conference. The new league was made up of Keokuk High School, Fort Madison High School, Mt. Pleasant High School, Fairfield High School, Oskaloosa High School, Ottumwa High School, and Washington High School. Oskaloosa left the conference after the 1983 baseball season to join the South Central Conference. Oskaloosa returned after the 1995-1996 School year after being asked to leave the South Central Conference due to enrollment numbers and after being denied membership into the Little Hawkeye Conference due to travel distance.
The name was later changed to Rosenwald High School. In 1980, Rosenwald High was partnered with the mostly white Poydras High School as part of a desegregation plan similar to that involving Batchelor High School and Innis High School. After Poydras High closed in 1981, Rosenwald became the only public high school in New Roads, and the only magnet high school in the parish. It was a high school in the Pointe Coupee Parish Public School system until after the 1990–91 school year, when the high school was closed down as part of a plan to combine all parish public schools into the newly formed Pointe Coupee Central High School.
Indian Hill High School is a public high school near Cincinnati, Ohio. It is the only high school in the Indian Hill Exempted Village School District.
