The Bulgarian Bag breaks the tradition with static resistance devices such as free weights which adhere to a singular plane of motion (i.e. creating resistance by pushing or pulling weight away from and toward the body), by using accelerating and deceleration movements to swing and spin the bag at various angles to athlete's body. This results in the Bulgarian Bag’s ability to increase overall body strength and agility. The multi-angular approach to gravity, momentum and inertia in physical exercise has been termed Variable Angular Resistance training in some use.

For those with wide thighs, such as bodybuilders with large quadriceps, or people with large amounts of fat stores on the inner thigh, it may cause chafing. If worn on both legs, however, the chafing would be between the weights and only damage them, possibly only chafing with a lack of tightness. Thigh weights are not ideal for movements involving quadriceps use, as they require tightness which may limit the use of the muscle and blood flow to the legs. For such movements, weights worn higher on the body or free weights are more ideal.

The great utility of the isoinertial method and at the same time what makes it different from the normal isotonic muscle movement lies in the fact that the action isotonic developed in conventional exercises (strength machines and free weights), the resistance is constant throughout the whole of movement in both the concentric phase in which the eccentric, which is equivalent to the set load. In the isoinertial method the resistance is adapted in every moment and is proportional to the force developed in the sense that the greater the force expressed by the subject in the concentric phase, the greater will be the acceleration with which the flywheel reacts.
These attributes all vary widely between the 30 boxers featured in the game (Evander Holyfield has the maximum rating on all of them). During career mode, all of the attributes can be increased by selecting various training activities, such as free weights sessions or a protein diet. Many of these portray somewhat unusual ways for a fighter to attempt to improve their boxing ability; for example, taking Karate lessons, paying a visit to a health club, or obtaining an iron gum shield. In exhibition mode, the player can choose to pit any boxer against any other, with the option to make each fighter controlled by either a player or the console.
