Frederick Matthias Alexander (1869–1955) was a Shakespearean orator from Tasmania, who developed voice loss during his unamplified performances. After doctors found no physical cause, Alexander reasoned that he was inadvertently damaging himself while speaking. He observed himself in multiple mirrors and saw that he was contracting his posture in preparation for any speech. He hypothesized that a habitual conditioned pattern (of pulling his head backwards and downwards) needlessly was disrupting the normal working of his total postural, breathing, and vocal processes.

As the concave bases of the arytenoid cartilages move on the two convex articular surfaces on the cricoid cartilage (at the cricoarytenoid articulations), the vocal processes are brought closer to each other, which permits the vocal folds to make contact (adduct) and abduct. It was discovered via stroboscope that during ordinary phonation, or speaking, in a man, the vocal folds contact with each other completely during each vibration, closing the gap between them fully, if just for a small length of time. This closure cuts off the escaping air. When the air pressure in the trachea rises as a result of this closure, the folds are blown apart, while the vocal processes of the arytenoid cartilages remain in apposition.

Intralaryngeal BOTOX injections bind specifically and non- competitively to presynaptic cholinergic neuron membranes at neuromuscular junctions which induce zinc-dependent cleavage of proteins involved in neuroexocytosis. The breakdown of neuroexocytosis proteins block acetylcholine secretions which inhibit hypertonicity, strengthen antagonist muscles and restore the balance of forces. Since laryngeal granuloma formations are exacerbated by repeated forceful contraction of the glottis, the combined effects of the toxin induce thyroarytenoid paresis and decreases the force of vocal fold adduction which inhibit forced contact between vocal processes, hence facilitating granuloma resolution. Oral zinc sulfate treatments are advantageous due to their ability to preserve the anatomical and functional integrity of the vocal cords.

Unlike other types of vocal production, it is difficult to film the vocal cords while they are operating in this manner as the epiglottis closes down over the larynx and the resonating chamber assumes its smallest dimensions. It is known that when producing pitches in this register, vibration occurs only in some anterior portion of the vocal folds. This shorter vibrating length naturally allows for easier production of high pitches. Although the whole physiological production of whistle tone is not understood, it is known that when the laterals are active but the transversus inactive, a triangular opening is seen between the arytenoids, the vocal processes contact each other, but the posterior parts at the apex do not contact each other.
