The police took them to Bromma airport in Stockholm, and then stood aside as masked alleged CIA operatives cut their clothes from their bodies, inserted drugged suppositories in their anuses, and dressed them in diapers and overalls, handcuffed and chained them and put them on an executive jet with American registration N379P.

The film follows much of the plot laid out by the novel, with some changes. In the film, Mark Renton is a 26-year-old, unemployed man who lives with his parents in Edinburgh, engaging in heroin use with Sick Boy (Jonny Lee Miller), Spud (Ewen Bremner) and their dealer Swanney "Mother Superior" (Peter Mullan). Begbie (Robert Carlyle) and Tommy (Kevin McKidd) urge Renton to cease heroin use and get clean, which Renton unsuccessfully tries to do with the use of opium suppositories. One night at a club, he meets a girl called Diane (Kelly Macdonald), who he later has sex with at her house.

Before the 1996 merger with Ciba-Geigy to form Novartis, Sandoz Pharmaceuticals (Sandoz AG) was a pharmaceutical company headquartered in Basel, Switzerland (as was Ciba-Geigy), and was best known for developing drugs such as Sandimmune for organ transplantation, the antipsychotic Clozaril, Mellaril Tablets and Serentil Tablets for treating psychiatric disorders, and Cafergot Tablets and Torecan Suppositories for treating migraine headaches. The Chemiefirma Kern und Sandoz ("Kern and Sandoz Chemistry Firm") was founded in 1886 by Alfred Kern (1850–1893) and Edouard Sandoz (1853–1928). The first dyes manufactured by them were alizarinblue and auramine. After Kern's death, the partnership became the corporation Chemische Fabrik vormals Sandoz in 1895.

The only track on which both Pierce and Kember appear is "When Tomorrow Hits", a cover of a Mudhoney song, originally intended for a double A-side split single, with Mudhoney covering "Revolution" from Playing With Fire. This release was scotched when Kember caught wind of the fact that Mudhoney had fitted "Revolution" with somewhat irreverent lyrics about methadone suppositories. The Mudhoney recording eventually surfaced as a b-side. There's a subtle continuity between both tracks, specifically duelling references to The Stooges; the Spacemen 3 track opens with the "look out!" invocation that began "Loose", and "When Tomorrow Hits" is mostly a rewrite of "I Wanna Be Your Dog".

An August 13, 2007, story by Jane Mayer in The New Yorker reported that the CIA has operated "black site" secret prisons by the direct presidential order of George W. Bush since shortly after 9/11, and that extreme psychological interrogation measures based at least partially on the Vietnam-era Phoenix Program were used on detainees. These included sensory deprivation, sleep deprivation, keeping prisoners naked indefinitely and photographing them naked to degrade and humiliate them, and forcibly administering drugs by suppositories to further break down their dignity. According to Mayer's report, CIA officers have taken out professional liability insurance, fearing that they could be criminally prosecuted if what they have already done became public knowledge.

The Untold Story "is not as slick as Gore Vidal's Caligula" and says that "the lack of gloss actually benefits the film, putting the subject matter in a very suitable light" and giving more of an impression of a "gritty documentation" compared with the "pretentious and almost glorifying" way in which the earlier film presented its subject matter. Stine also mentions the "gratuitous gore" - in particular the "tongue-sawing sequence" and "the poor chap who gets impaled with a spear in a manner that makes suppositories look positively quaint". In his D'Amato monograph published in 2004, Gordiano Lupi calls the film "crazy and perverse" as well as "visionary in the right place".
