The top kill procedure was used to plug flaming oil wells, blown up by retreating Iraqi forces, in 1991, during the Gulf War. This has caused spills of large quantities of oil and fires on drilling rigs. The blowout of the Lucas well at the Spindletop field in 1901 lasted for over nine days and spilled over of oil. James Smither Abercrombie (1891–1975), a Texas oil driller, and Harry S. Cameron (1872–1928), who operated a machine shop, formed the Cameron Iron Works in 1920. Abercrombie had an idea for a blowout preventer and took it to Cameron.
Secondary oil well control is done after the Primary oil well control has failed to prevent formation fluids from entering the wellbore. This process uses "blow out preventer", a BOP, to prevent the escape of wellbore fluids from the well. As the rams and choke of the BOP remain closed, a pressure built up test is carried out and a kill mud weight calculated and pumped inside the well to kill the kick and circulate it out.
Each of these inspections had taken two hours or less. During its lifetime the rig received 5 citations for non-compliance, 4 of which were in 2002 (safety, including the blowout preventer) and the other in 2003 (pollution). Braided line is more complex than slickline due to the need for a grease injection system in the rigup to ensure the blowout preventer (BOP) can seal around the braided contours of the wire. It also requires an additional shear-seal BOP as a tertiary barrier as the upper master valve on the Christmas tree can only cut slickline. Braided line includes both the core-less variety used for heaving fishing and electric-line used for well logging and perforating.
When jibing a fully loaded mainsail in a following sea, the following procedure may be used. Using the steering, the stern of the boat is carefully brought up into the wind. Then the leeward, working preventer is released little by little, while the mainsheet is shortened to bring in the boom. BP drilled two relief wells into the original well to enable them to block it. Once the relief wells reached the original borehole, the operator pumped drilling fluid into the original well. Transocean's Development Driller III started drilling a first relief well on May 2 and was at out of as of June 14. GSF Development Driller II started drilling a second relief on May 16 and was halted at out of as of June 14 while BP engineers verified the operational status of the second relief well's blowout preventer.

The J.J. Young 34-2 #1 well in St. Clair County, Alabama (Alabama State Oil and Gas Board Permit No. 4325) was drilled as a joint venture between Amoco and ARCO. This well was spudded on October 30, 1984. The well was initially drilled with air to approximately when it encountered a significant quantity of gas that reportedly required closing the blowout preventer. The engineers for both Amoco and Arco said that the J. J. Young #1 would flow over one million feet3 of gas per day (over 28,000 m3).
Remotely operated underwater vehicles (ROVs) would be dispatched to inspect the condition of the wellhead, Blowout Preventer (BOP) and other subsea well equipment. The debris removal process would begin immediately to provide clear access for a capping stack. Once lowered and latched on the wellhead, a capping stack uses stored hydraulic pressure to close a hydraulic ram and stop the flow of hydrocarbons. If shutting in the well could introduce unstable geological conditions in the wellbore, a cap and flow procedure would be used to contain hydrocarbons and safely transport them to a surface vessel.
Despite acknowledging the pressure reading and agreeing that it was worrisome, BP's supervisors did not run the test again. Ordering a retest would have begun with the closing of the rig's blowout preventer, which would have stopped the flow before the blowout. DOJ attorneys state, "That such a simple, yet fundamental and safety-critical test could have been so stunningly, blindingly botched in so many ways, by so many people, demonstrates gross negligence." The government cited "a culture of corporate recklessness" in their investigation of the events leading up to the blow out.

Before any workover, the well must first be killed. Since workovers are long planned in advance, there would be much time to plan the well kill and so the reverse circulation would be common. The intense nature of this operation often requires no less than the capabilities of a drilling rig. The workover begins by killing the well then removing the wellhead and possibly the flow line, then installing a B.O.P commonly known as a blowout preventer, then lifting the tubing hanger from the casing head, thus beginning to pull the completion out of the well.

While this technique had worked in shallower water, it failed here when gas combined with cold water to form methane hydrate crystals that blocked the opening at the top of the dome. Pumping heavy drilling fluids into the blowout preventer to restrict the flow of oil before sealing it permanently with cement ("top kill") also failed. BP then inserted a riser insertion tube into the pipe and a stopper-like washer around the tube plugged at the end of the riser and diverted the flow into the insertion tube. The collected gas was flared and oil stored on board the drillship Discoverer Enterprise.
The US Coast Guard had issued pollution citations for Deepwater Horizon 18 times between 2000 and 2010, and had investigated 16 fires and other incidents. These incidents were considered typical for a Gulf platform and were not connected to the April 2010 explosion and spill. The Deepwater Horizon had other serious incidents, including one in 2008 in which 77 people were evacuated from the platform when it tilted and began to sink after a section of pipe was incorrectly removed from the platform's ballast system. On 10 May 1977, the crew on board the drill rig Venture One was preparing to lower a Blow Out Preventer (BOP) to the seabed below. International Underwater Contractors (IUC) diving supervisor Richard Pettit had been asked to inspect the Permanent Guide Base to verify that it was clear of any obstructions that might prevent the installation of the BOP. At approximately 0400, Pettit launched the bell with divers Dave Hammond, 29, and Craig Hoffman, 22, inside. Described as a “very competent diver,” Hoffman was from the state of Delaware and had worked for IUC in America before coming to the North Sea.
The Deepwater Nautilus, sister rig to the Deepwater Horizon being transported aboard a heavy-lift ship Deepwater Horizon was a fifth-generation, RBS-8D design (i.e. model type), deepwater, dynamically positioned, column-stabilized, semi-submersible mobile offshore drilling unit, designed to drill subsea wells for oil exploration and production using an , blowout preventer, and a outside diameter marine riser. Deepwater Horizon was the second semi-submersible rig constructed of a class of two, although Deepwater Nautilus, its predecessor, is not dynamically positioned. The rig was and capable of operating in waters up to deep, to a maximum drill depth of .
Deepwater Horizon in flames after the explosion At CDT on 20 April 2010, during the final phases of drilling the exploratory well at Macondo, a geyser of seawater erupted from the marine riser onto the rig, shooting into the air. This was soon followed by the eruption of a slushy combination of drilling mud, methane gas, and water. The gas component of the slushy material quickly transitioned into a fully gaseous state and then ignited into a series of explosions and then a firestorm. An attempt was made to activate the blowout preventer, but it failed.
On July 15, 2010, BP announced that it had successfully plugged the oil leak using a tightly fitted cap. The cap, weighing 75 tons and standing high, is now bolted to the failed blowout preventer. It consists of a Flange Transition Spool and a 3 Ram Stack and is a temporary solution. President Barack Obama cautiously welcomed the news that the leak had been stopped, though stressing, "it is important we don't get ahead of ourselves". If the cap holds for the planned 48 hours, its valves will be reopened temporarily in order to allow for seismic tests to ensure that oil is not escaping into the bedrock.
By 26 May 2010, over 130 lawsuits relating to the spill had been filed against one or more of BP, Transocean, Cameron International Corporation, and Halliburton Energy Services, although it was considered likely by observers that these would be combined into one court as a multidistrict litigation. On 21 April 2011, BP issued $40 billion worth of lawsuits against rig owner Transocean, cementer Halliburton and blowout preventer manufacturer Cameron. The oil firm alleged failed safety systems and irresponsible behaviour of contractors had led to the explosion, including claims that Halliburton failed to properly use modelling software to analyze safe drilling conditions. The firms deny the allegations.

The vessel was used to position the large containment dome and also to attempt the "top kill" in which large quantities of drilling mud were pumped down the Q4000 drilling pipe into the failed Deepwater Horizon blowout preventer valve (BOP) in an attempt to stop the flow of oil. Both attempts failed to stop the oil leaking from the well.
According to the US Congressional investigation, the rig's blowout preventer, built by Cameron International Corporation, had a hydraulic leak and a failed battery, and therefore failed. On 8 September 2010, BP released a 193-page report on its web site. The report places some of the blame for the accident on BP but also on Halliburton and Transocean. The report found that on 20 April 2010, managers misread pressure data and gave their approval for rig workers to replace drilling fluid in the well with seawater, which was not heavy enough to prevent gas that had been leaking into the well from firing up the pipe to the rig, causing the explosion.
On July 10, the containment cap was removed to replace it with a better-fitting cap ("Top Hat Number 10"). Mud and cement were later pumped in through the top of the well to reduce the pressure inside it which didn't work either. A final device was created to attach a chamber of larger diameter than the flowing pipe with a flange that bolted to the top of the blowout preventer and a manual valve set to close off the flow once attached. On July 15, the device was secured and time was taken closing the valves to ensure the attachment under increasing pressure until the valves were closed completing the temporary measures.
The Department of the Interior exempted BP's Gulf of Mexico drilling operation from a detailed environmental impact study after concluding that a massive oil spill was unlikely. In addition, following a loosening of regulations in 2008, BP was not required to file a detailed blowout plan. The BP wellhead had been fitted with a blowout preventer (BOP), but it was not fitted with remote- control or acoustically activated triggers for use in case of an emergency requiring a platform to be evacuated. It did have a dead man's switch designed to automatically cut the pipe and seal the well if communication from the platform is lost, but it was unknown whether the switch was activated.
Internal BP documents show that BP engineers had concerns as early as 2009 that the metal casing that BP wanted to use might collapse under high pressure. According to a number of rig workers, it was understood that workers could get fired for raising safety concerns that might delay drilling. In March 2010, the rig experienced problems that included drilling mud falling into the undersea oil formation, sudden gas releases, a pipe falling into the well, and at least three occasions of the blowout preventer leaking fluid. The rig's mechanic stated that the well had been experiencing problems for months, and that the drill repeatedly kicked due to resistance from high gas pressure.
VetcoGray operated in around 30 countries and was one of the world's leading suppliers of drilling, completion and production equipment for on- and offshore oil and gas fields, including subsea applications. In October 2007, GE acquired Sondex Plc, a UK-based oilfield services technology company that specialized in the engineering and manufacturing of directional drilling and formation evaluation systems and wireline tools for the production of oil and gas, for £290 million.
