Medical errors are the third leading cause of death in the US, after heart disease and cancer, according to research by Johns Hopkins University. Their study published in May 2016 concludes that more than 250,000 people die every year due to medical mix-ups.
Justice Minister Wickbom was forced to resign after Bergling's escape. Wickbom had claimed that he had not been informed that Bergling had been granted regular conjugal visits and that his escape was a result of a series of mix-ups between the police and the prison service. The information about Bergling's conjugal visit was at the Department of Justice at the time of the escape, though he had not seen it. Prime Minister Ingvar Carlsson said he accepted Wickboms departure, but praised his courage in his decision to resign.
The wealthy girl Esmeralda (Theda Bara) is kidnapped by gypsies at birth and becomes, as one might assume, the darling of Paris. She is loved by the bell ringer and former hunchback Quasimodo (Glen White), Frollo (Walter Law), the wicked surgeon who cares for him, and an equally wicked Captain Phoebus (Herbert Heyes). However, the titular hunchback is downplayed in favor of gypsy dancing girl Esmerelda. The surgeon kills the Captain and frames Esmeralda, but after many merry mix-ups, she winds back with her wealthy family, happily wed to Quasimodo.
Her new default outfit is based on those worn by Japanese idols and she is wearing a red headband out of respect for Ryu. Sakura is controlled in the same way as Ryu and Ken, but her special moves flow differently and are less powerful. Sakura's moveset contains many variants of their moves, including a running Shoryuken, a jumping Tatsumaki, and a less-powerful Hadouken that she can charge, though at the cost of distance. Her moves and combos have high potential to stun opponents and also provide many an opportunity for mix-ups.
There are also some other films that have been released as sequels to various films in Romero's Dead series, most likely to ride on the name recognition that Romero's films enjoy. They have been produced due to the various mix-ups with the copyright and ownership of the movies, Romero himself owns only Dawn of the Dead from his first four films. Romero is often positive towards derivations of his work, stating that any new film in the horror genre is a step forward, whether completely original or a 'copycat'.
In June 2016, the Polish government announced it would re-open the investigation into the Smolensk jet crash with plans to exhume and autopsy all 96 of the victims. On 14 November 2016, the first of ten bodies, including Kaczyński's, were exhumed. Kaczyński and his wife were reburied on 18 November 2016 after autopsies. By 1 June 2017, exhumations of 27 coffins had been completed and DNA tests confirmed that 24 of those coffins, Kaczyński's among them, showed evidence of mix-ups, including switched bodies, partial sets of remains and multiple remains in one grave.
The home kit featured a subtle checkerboard print on the shirt and was very worn with black socks, last seen between mid 1920s until 1957. The away kit was unsurprisingly white with claret shorts and white socks – and has the same design as the home kit. Claret and sky blue shorts/socks were worn in several away games in order to prevent kit mix-ups. The three goalkeeper kits were blue, black and grey and was based on Nike's 2011 template which featured stunning details on the sleeves and the side of the shirt.
On February 23, three interviews were released on the website - one each with composer Tuomas Holopainen, director Stobe Harju, and producer Markus Selin. Holopainen revealed the origin of the project, and that Nightwish will appear as themselves in the film, with minor roles. On August 31, the band announced that the album and movie had changed name from Imaginarium to Imaginaerum, to avoid mix-ups with other things entitled "Imaginarium". This also led to changing the title of the track with the same name, but the song "Storytime" still includes the word "imaginarium" in the lyrics.
Unfortunately, the bomb explodes prematurely and the trio escapes. Vinod and Sudhir find out that the body is with Tarneja's rival, Ahuja who had, in an inebriated condition, carried the coffin to his farmhouse. They steal the corpse but not before Tarneja, Ahuja, the new Municipal Commissioner Srivastav, Shobha and others also get involved resulting in a series of comic mix-ups. The climax is set upon a stage dramatization of the Mahabharata, particularly the enactment of the Draupadi Cheer-Haran episode, which is turned on its head with the duo and the group following them inserting themselves into the scene.
Poster, Mystery Writers Key West Fest, June 2014 Griffin's knowledge of military jargon and administrative writing style shows when fictional orders and dispatches are incorporated in his novels. Many of his characters must battle red tape and bureaucratic mix-ups, sometimes making humorous end-runs around the system. Griffin was the co-founder of the William E. Colby Seminar on Intelligence, Military, and Diplomatic Affairs at Norwich University in Vermont, along with his friend, historian, and Patton biographer Colonel Carlo D'Este. Griffin was a member of the Colby Circle, having participated in the William E. Colby Writers Symposium at Norwich University.
According to police, two witnesses claimed to have last seen him on a trail near Shanty Bay, Ontario. The first reported that he had seen him having mechanical trouble with his bicycle on the trail. The second witness reported seeing Crisp later walking along the trail further east, but due to communications mix-ups this information was not passed along to searchers until the end of October. Crisp's missing person file was flagged nationally by the Canadian Police Information Centre. Crisp's parents reported him to Barrie Police Service as missing on the morning of October 14.
Examples include cases of an adopted child attempting to reunify with his or her biological mother, potential hospital mix-ups, and in vitro fertilization where the laboratory may have implanted an unrelated embryo inside the mother. Other factors, such as new laws regarding reproductive technologies using donated eggs and sperm and surrogate mothers, can mean that the female giving birth is not necessarily the legal mother of the child. For example, in Canada, the federal Human Assisted Reproduction Act provides for the use of hired surrogate mothers. The legal mother of the child may be the egg donor.
The two stations near Culworth and the surrounding villages and hamlets lay on different lines and served different purposes. And it has to be said, that with such obliquely given names, there were mix-ups. Culworth station only catered for the smaller stations on the north–south axis, including the neighbouring town of Brackley: for more distant destinations, a change had to be made at Woodford & Hinton or Brackley. Eydon Road Halt station on the Banbury Branch was used to reach the larger town of Banbury or the village of Woodford Halse - at either of which a connection could be made with trains running further afield.
On August 22, 2011, it was announced on the band's official website that the album of Imaginaerum would be released at the end of the year. On August 31, Nightwish announced on its website the decision to change Imaginarium title to Imaginaerum "to avoid mix-ups with various things named 'Imaginarium'". On September 2, the group announced on its website that the new single called "Storytime" would be released on Friday, November 11, and on September 9, Nightwish revealed the cover and tracklist. Nightwish performing in Denver, Colorado with Elize Ryd and Alissa White-Gluz, who filled in for Anette Olzon, who parted ways with the band afterwards.

Millar, as Violet, and boys The Orchid is an Edwardian musical comedy in two acts with music by Ivan Caryll and Lionel Monckton, a book by James T. Tanner, lyrics by Adrian Ross and Percy Greenbank, and additional numbers by Paul Rubens. The story concerns marital mix-ups and the quest of a wealthy man for a $2,000 Peruvian orchid to be sent to France. When foul play keeps the flower from reaching its destination, it is discovered that a nearly identical orchid is growing in the garden of the horticultural college. The musical opened on 26 October 1903 at London's Gaiety Theatre and ran for 559 performances.
Colossal sued the Rock & Roll Hall of Fame for $1,200,000 in damages plus $10,000,000 in punitive damages. That year, Colossal Pictures signed a development deal with the Disney Channel to produce content for the network. (C)P produced the interstitial series Frankenguy & The Professor and The Mix-ups plus the Zoog Disney block for the Disney Channel. After (C)P decided to restructure itself into a smaller company, consolidating all of its activities into one building in the process, they emerged from Chapter 11 bankruptcy on December 1, 1997. Jamie Hyneman, manager of Colossal Pictures’ model shop, took over the facility and turned it into M5 Industries.
Based on the MIT confusion matrix of 1620 single phoneme errors, the phoneme with the greatest margin of speech error is l [l] mistaken for r [r]. Other phonemes that had a high level of speech error include s [s] mistaken for ʃ [sh], f [f] for p [p], l [l] for r [r] and r [r] for l [l], w [w] for r [r], and many more. These sounds are most likely to transform to a similar sound when placed in near vicinity of each other. Most of these mix-ups can be attributed to the two phonemes having similar areas of articulation in the mouth.
Copies numbered 145 and 974 of the SKM were retained by the Russians while was dispatched from Scapa Flow to Alexandrovosk in order to collect the copy offered to the British. Although she arrived on 7 September, due to mix-ups she did not depart until 30 September and returned to Scapa with Captain Kredoff, Commander Smirnoff and the documents on 10 October. The books were formally handed over to the First Lord, Winston Churchill, on 13 October. The SKM by itself was incomplete as a means of decoding messages, since they were normally enciphered as well as coded and those that could be understood were mostly weather reports.
The description of Solatisonax cabrali by Tenório and colleagues, in 2011, was initially based on four young and eroded juvenile shells. The subsequent discovery of two well-preserved empty shells from southeastern Brazil (a juvenile and an adult specimen) brought new information on the species' distribution, ontogeny and morphology, which culminated in a redescription by Cavallari and colleagues in 2013. A few mix-ups involving type specimen figuration and measurements in the original description were also stressed and rectified in the redescription. S. cabrali was named after Brazilian government employee Enilson Cabral, an analyst of the Brazilian Institute of Environment and Renewable Natural Resources (IBAMA) responsible for collecting the materials used in Tenório's study.
The land where Rubonia was laid out was part of an 80-acre plot that Albert Stonelake, a United States Union Army surgeon, purchased in 1868. Stonelake sold the undeveloped land to Marcus DeVoursney in 1881. DeVoursney died in 1904 and his estate sold the land in 1911 to William and Nellie Smith, who platted the area in 1913 with plans to develop it as a neighborhood known as East Terra Ceia. The area was planned as housing for African Americans working in the area as migrant farmers. Within a few years, Atlantic Coast Line Railroad officials requested that East Terra Ceia’s name be changed due to frequent mix-ups between it and adjacent Terra Ceia.
The Rangers' half of the eighth featured a series of bullpen mix-ups by the Cardinals, leaving Tony La Russa without closer Jason Motte in a crucial situation. After Michael Young led off the inning with a double, La Russa sent both Motte (a right-hander) and left-handed reliever Marc Rzepczynski to begin warmups. However, Cardinals bullpen coach Derek Lilliquist later stated that he only heard Rzepczynski's name called. When La Russa saw that Motte was not warming up, he made a second call to the bullpen, but this time Lilliquist thought he heard La Russa call for reliever Lance Lynn, who was supposedly unavailable for the game due to throwing 47 pitches in Game 3.
"This Week in Fiction: Shirley Jackson." NewYorker.com. Retrieved 2018-07-29. The book relays a series of small comical adventures largely contrasting the children's natural acceptance of the change with their parents' struggles to keep up with them, such as eldest child Laurie's introduction to kindergarten (and his daily reporting of troublemaker classmate Charles' antics); middle child Jannie's insistence that her seven imaginary daughters (who all have the same name) be taken into account on every family outing; the comedy of the family's third child, Sally, whose lengthy delay in being born throws the whole family into chaos; and the night the entire family came down with grippe and the ensuing mix-ups.
The series begins after Marcy Bradford (played by Emma Samms in the second- season episode "In Her Dreams"), the mother of 12-year-old Nicole Bradford (Keanan), has died. The two men who had competed for Marcy's affections in Key West during the summer of 1974 – Michael Taylor (Reiser), a successful financial advisor, and Joey Harris (Evigan), a struggling artist, former friends who grew to hate one another over their mutual interest in the woman – are awarded joint custody of Nicole. Each week, the mix-ups and trials of two single men raising a teenage daughter provide the stories. Judge Margaret W. Wilbur (Florence Stanley), a family court judge who gave custody of Nicole to Michael and Joey, frequently visits the new family.
Morris' productivity declined in the latter half of his career, something he put down to the break-up of his opening pairing with Barnes. Morris' partnerships with his later partners yielded less runs, leading him to remark that "When Siddy [Barnes] went, I lost a lot of support because he'd always get ones." Morris was also known for his unselfishness, often sacrificing his wicket after being involved in mix- ups while running between wickets, and he had a reputation for not attempting to finish not out to inflate his average. However, Morris was regarded as the "bunny" of English medium pace bowler Alec Bedser,McHarg, p. 14. who dismissed him 20 times in first-class cricket, including 18 times in Test matches.
Back at the Luau, Abby tells Jeff about the rumor that Gidget sleeps around, admitting that it's a lie and that Gidget only had one affair with a guy named "Moondoggie". Jeff then realizes how much he cares about Gidget, so he puts Abby in her place by telling her to call him what everyone at home calls him: "Moondoggie". Jeff and Gidget reconcile on the beach and head back to the hotel to straighten everything out with the adults. Through a few mix-ups, Gidget believes her parents are cheating on each other with Abby's parents, and Gidget's parents believe Gidget's gone missing and alert all of her friends, including Abby who deeply regrets the trouble she has caused.
Eventually, the twins meet and discover (Twin telepathy) that they have a strange connection with each other. As a result, a string of hilarious mix-ups ensues when Ma Yau is accidentally enlisted by the gangsters to participate as an escape driver in the liberation of none other than Crazy Kung; Bok Min in turn is forced to conduct Yau's concert (which becomes a smash hit despite him having absolutely no musical talent); and the two of them end up with the other's girl as their respective love interest. Eventually, things come to a head when Tarzan is kidnapped by the gangsters. The twins join up to defeat the gang that has turned their lives upside down, and in a showdown in a vehicle testing center.
An enlargement of a photograph of the U.S. Capitol taken by KH-7 mission 4025 on 19 February 1966. A KH-7 GAMBIT optical reconnaissance satellite. :Section source: Space Review Gambit marked the first use of next-generation launch vehicle systems as Convair and Lockheed, the builders of the Atlas-Agena booster, began introducing improved, standardized launchers to replace the multitude of customized variants used up to 1963, which caused endless mix ups, poor reliability, and mission failures. This followed a recommendation by the Lewis Spaceflight Center in Cleveland, Ohio that Atlas and Agena switch to one standard configuration for both NASA and Air Force launches, with uniform testing and checkout procedures, as well as improved materials and fabrication processes for the various hardware components in the boosters.
For instance, he left a small inked, coded signature beside the incisions of some of his surgical patients. He also proposed tattooing mothers and newborn babies with matching marks to avoid accidental mix ups. Kane was a member of a notable Pennsylvania family that included several physicians (including his mother) that had earlier given its name to both their community and his primary hospital of practice; their family home, Anoatok, is now on the National Registry of Historic places. Kane was also in the public eye in 1931 when he testified at the sensational trial of his son, Elisha Kent Kane III, a college professor, who was acquitted of murder in Elizabeth City County, Virginia after the drowning death of his wife during their trip to a Back River Light beach.
Due to this success, in his autobiography, Charles wrote that "signing for Arsenal was the most terrible choice I ever made". Though he also wrote that "I'll always have a place in my heart for Arsenal, and it was an honour to have played for them." He was paid £20 a week, and set up with a house and car by the Arsenal management – a Sunbeam-Talbot with the license plate 'MEL 9'. Not used to the complexities of a rapidly modernising game, he endured a frosty relationship with coach Ron Greenwood after numerous humorous mix-ups over new terms such as 'blind-side run' and 'marking space', and embarrassed himself in front of the TV cameras when he fumbled and said "I'm okay, I've just got clitorises in my eyes", instead of cataracts.
According to The New York Times, corporate communications shifted from a monologue to two-way conversational communications and new media also made it "easier for consumers to learn about the mix-ups and blunders" of PR. For example, after the Deepwater Horizon oil spill, BP tried to deflect blame to other parties, claim the spill was not as significant as it was and focused on the science, while human interest stories related to the damage were emerging. In 2011, Facebook tried to covertly spread privacy concerns about competitor Google's Social Circles. Chapstick created a communications crisis after allegedly, repeatedly deleting negative comments on its Facebook page. During the Iraq War, it was exposed that the US created false radio personalities to spread pro-American information and paid Iraqi newspapers to write articles written by American troops.
There are two reasons for the flow increase: (1) the area of the port is larger, due to the dog leg and (2) the shape of the port floor was changed from a concave to a convex curve. The concave floor tended to bend the exhaust flow upwards which caused turbulence when the flow was forced to go down into the exhaust manifolds. By switching to a convex floor the curvature of the flow starts in the head and proceeds much more smoothly into the exhaust manifold resulting in less turbulence and better flow. The center two intake bolts on each head were relocated to prevent accidental mix-ups of Gen-2 and Gen-3 intakes. The intakes can be interchanged by slotting the bolt holes, but the added deck height of the GEN-3 engine means that sealing and port match will be compromised.
Artist Tavar Zawacki painted a site-specific wordplay painting in Lima, Peru, commenting on the cocaine crisis and exportation Word play or wordplay (also: play-on-words) is a literary technique and a form of wit in which words used become the main subject of the work, primarily for the purpose of intended effect or amusement. Examples of word play include puns, phonetic mix-ups such as spoonerisms, obscure words and meanings, clever rhetorical excursions, oddly formed sentences, double entendres, and telling character names (such as in the play The Importance of Being Earnest, Ernest being a given name that sounds exactly like the adjective earnest). Word play is quite common in oral cultures as a method of reinforcing meaning. Examples of text-based (orthographic) word play are found in languages with or without alphabet-based scripts, such as homophonic puns in Mandarin Chinese.
Destinos recounts the story of Los Angeles-based lawyer Raquel Rodríguez who is hired by the family of Fernando Castillo (Augusto Benedico). He had discovered that his first wife, Rosario, did not die in the Spanish Civil War as he had believed, but had survived and had an unknown child. In the course of her investigation of the case, Raquel travels to a number of Spanish-speaking areas -- Seville and Madrid, Spain; Buenos Aires, Argentina; San Juan and San Germán, Puerto Rico; and Mexico -- has a number of adventures and mishaps, meets a love interest (Arturo Puig) and faces a number of melodramatic conflicts. Recurring plot elements include Raquel traveling in pursuit of the investigation, letters (primarily to determine last known addresses of people), comic mix-ups, and death (although nobody dies during the series, many characters are found to have died and Don Fernando is on the verge of dying).
Information drawn from the IPF catalog. The 1948 postage dues were denominated 3, 5, 10, 20, and 50 m. with the 1948 Doar Ivri design. The tabs have gone through three unofficial phases. From 1948 to 1954, the tabs were written in Hebrew (with four exceptions: the Maccabia, Israel Bonds, Zionist Congress and Z.O.A. stamps). From 1954 until 1967, the inscriptions were usually in Hebrew and French. Since 1967, the tabs are typically Hebrew and English. Rarely, a tab is matched with the wrong stamp, as with two mix-ups on some Doar Ivri stamps. From the outset, Israel created its own commemorative cancellations, including a first day cancel for the new Doar Ivri on May 16, 1948, and cancels for the Maccabiah Games and its major cities the same year.Israel Philatelic Federation cancels list By 1960, more than 325 unique postmarks had been designed.
When bogie carriages were added to the fleet, they initially were allocated vacant numbers in the appropriate class series, one of the first examples being 70 A. From 1886 the bogie cars were re-coded as AA, BB, or ABAB; it is thought that this was necessary due to mix-ups with carriage capacities. Twenty seven bogie carriages of the American end-loading saloon design had been built for the Victorian Railways between 1874 and 1887 (thereafter, so-called dog box bogie cars were built), along with at least one bogie double-saloon car inherited from the former private suburban railway company (those cars were re-classed about the same time). As bogie carriages displaced their fixed-wheel predecessors, the older cars were reassigned to other duties. In particular, with the majority of bogie carriages being first- class, first-class fixed-wheel vehicles were downgraded to second-class and re-coded as 'B'.
All 30 MLB teams will wear special colored jerseys with contrasting sleeves on the weekend of August 25–27, 2017, for Players Weekend. The Cardinals and Pirates will also use these jerseys on August 20, when they meet in the MLB Little League Game in Williamsport, Pennsylvania; both this game and Players Weekend coincide with the 2017 Little League World Series in South Williamsport. Notably, Players Weekend will mark the first time the Yankees have ever placed names on the back of their jerseys. The trend toward multiple uniforms of the same color can also lead to on-field mix-ups: in 2011, Brewers pitcher Zach Greinke, making a pinch- hitting appearance, accidentally wore the "Bierbrauer" jersey the night before German Heritage Day, while in 2014, just three games into the first season where the alternate was introduced, outfielder Junior Lake played the first inning in a "Chicago" jersey while the rest of the team wore "Cubs".
The series is about lazy, bungling, incompetent civil servants, "Number One" – Roland Hamilton-Jones (Wilfrid Hyde-White) and later Deryck Lennox-Brown (Deryck Guyler), "Number Two" – Richard Lamb (Richard Murdoch), with their dim, typo-prone, teenage secretary, Mildred Murfin (Norma Ronald), all watched-over by the lecherous, pompous, self-seeking Permanent Under-Secretary Sir Gregory Pitkin (Roy Dotrice and later Ronald Baddiley), all members of the British Civil Service based in Whitehall. The stories centred on their General Assistance Department (analogous to the "Department of Administrative Affairs" in the later Yes Minister), which helps other governmental departments. Instead of assistance, the department creates mix-ups, misunderstandings and cock-ups that lead to a telling-off from Sir Gregory, who sees his 'hard earned' Civil Service career and pension disappearing. In one 1960s episode, "The Big Rocket", General Assistance Department is put in charge of publicity for Britain's almost non- existent space programme.
The main Byzantine sources, George Akropolites, Nikephoros Gregoras, and George Pachymeres, offer considerably divergent accounts on the exact course of events before and during the battle, while the Western sources, chiefly the Greek and French versions of the Chronicle of the Morea and the history of the Venetian Marino Sanudo Torcello, in turn differ from the Byzantine sources and from each other. The narrative of the Chronicle is generally considered less reliable, being riddled with errors and mix-ups, but often provides details not appearing elsewhere. The account of the Chronicle focuses not on Prince William of Villehardouin, but on his nephew Geoffrey of Briel, whose deeds in the battle are presented in length, and in a style reminiscent of contemporary epic poems on Achilles or Digenes Akritas, whereas William is almost mentioned in passing. Briel was the only male grandchild of the first Villehardouin Prince of Achaea, Geoffrey I of Villehardouin, and hence a potential claimant to the Principality.
Ekman also acted in two films that would gain international recognition: F. W. Murnau's silent film classic Faust, where he played the title character, and in the original 1936 version of Intermezzo, where he played a world-famous violinist opposite Ingrid Bergman in her breakout role. But they had in fact already acted opposite each other in the Swedish 1935 movie Swedenhielms where they share a couple of wonderful scenes alone together as their characters have a heart-to-heart conversation on life and love; among the most memorable moments in the film. His best on-screen credit is often said to be his King Charles XV/Leonard Pettersson in film comedy Kungen kommer (1936), where Ekman masterfully plays a double-role in the film; first the king and then also the king's lookalike; an actor who is hired by members of the Royal Court to impersonate His Majesty at a private party. Naturally the real king later arrives at the party causing a number of confusions and comic mix-ups.
