To become "made", an associate first must be Italian or of Italian descent and sponsored by another made man. An inductee will be required to take the oath of omertà, the Mafia code of silence and code of honor. Nicolo Impastato was an admitted member of the Mafia who was born near Palermo, Sicily and became a Mafioso while still in Sicily. Joseph Charles Massino (born January 10, 1943) is an American former mobster. He was a member of the Mafia and boss of the Bonanno crime family from 1991 until 2004, when he became the first boss of one of the Five Families in New York City to turn state's evidence. Massino was a protégé of Philip Rastelli, who took control of the Bonanno family in 1973. Rastelli spent most of his reign in and out of prison, but was able to get the assassination of Carmine Galante, a mobster vying for power, approved in 1979.

After being captured and given a forcible penectomy by Russian mobsters, he was ostracized by his family as they felt him unworthy to succeed to its leadership. Starr has him torture Cassidy in Masada by shooting him multiple times with a Lee–Enfield rifle, then repeating the process after Cassidy regenerates. He is killed when Jesse punches him into the pit Cassidy is held in, causing him to break his neck and lie helpless as Cassidy drains him of his blood.

In 1962, De Mauro was the first to publish a detailed map of the Mafia, which was confirmed 22 years later by the Mafia pentito (turncoat) Tommaso Buscetta in his testimony to Judge Giovanni Falcone. In January 1962 he published a series of articles in L'Ora disclosing the testimony of Melchiorre Allegra, a medical doctor and a member of the Mafia from 1916 until his arrest in 1937. Upon being arrested, Allegra disclosed his membership and testified about Mafia activities. It was one of the first testimonies about the Mafia from within, but the document had been neglected until De Mauro republished it.

Palazzolo has always denied that he is involved in organised crime, has any links to the Mafia, nor enjoys close relations with politicians in the government. Gioacchino La Barbera (born November 1959 in Altofonte) is a member of the Mafia who became a pentito. He was one of the key witnesses in the trial against the killers of Antimafia judge Giovanni Falcone. La Barbera was born in Altofonte, in the province of Palermo. In 1981, he was initiated in the Altofonte cosca and, in 1986, he became the regent of the Altofonte Mafia family after the arrest of Bernardo Brusca. After the arrest of Mafia boss Totò Riina in January 1993, the remaining bosses, among them La Barbera, Giuseppe Graviano, Matteo Messina Denaro, Giovanni Brusca, Leoluca Bagarella, and Antonino Gioè, came together a few times (often in the Santa Flavia area in Bagheria, on an estate owned by the mafioso Leonardo Greco).

During his trial, it was found out he had recently been made a member of the Mafia. He admitted to the murder, claiming it was part of a power struggle between the various factions of the Patriarca family and that it was a case of "kill or be killed" - that if he didn't kill Grasso, "Wild Guy" would have killed him. In May 2012, Chow was secretly recorded by the FBI while he was conferring with "George" Heng Nieh (Chow's driver), stating that he did not want the CKT involved with "Jordan", the undercover agent posing as a member of the Mafia, for fear the CKT would be labeled 'an underworld society' that participated in organized crime. The case against Chow was built in part on the thousands of dollars given by "Jordan" to Chow; on one occasion, "Jordan" gave Chow $2,000 as thanks for the opportunity to work with Nieh. Although Chow protested, saying "Damn, that is bribery money, dude — that's not good", he did not return the payment. "Jordan"'s activities later expanded by December 2013 to include the trafficking of stolen liquor, cigarettes without a tax stamp, and marijuana.

In 1960, at the age of 19, Vitale was affiliated to the clan mafia of Altarello, which was led by his uncle said Giovanbattista "Titta". Vitale carried out extortions of construction companies on the orders of his uncle and his associate Giuseppe Calò, later being promoted to capodecina. In 1972, Vitale was arrested on suspicion of being implicated in the kidnapping of the manufacturer Luciano Cassina, but was released after a week of isolation in the prison of Asinara. Whilst held in custody, he had showed signs of depression that led to coprophagia and practiced self mutilation as his own act of contrition; doctors subjected him to electroconvulsive therapy. On 29 March 1973, Vitale walked into a Palermo police station and declared that he was a member of the Mafia and confessed to various acts of extortion, arson and two homicides.

In other cases, partially Italian-American associates have hidden their non-Italian heritage to become made men, as in the case of Scarfo crime family soldier and made man Andrew Thomas DelGiorno, who was of Polish and Italian descent but managed to conceal his Polish heritage on his mother's side and was therefore inducted into the Philadelphia Mafia. An associate of a crime family who has been in the police force or even attended a police academy usually cannot become a made member of the Mafia. For example, DeMeo crew member Henry Borelli could never become a made man in the Gambino family, since he had taken the New York City Police Department entrance exam in the early 1970s. Bonanno underboss Salvatore Vitale was only "made" because his brother-in-law and future boss Joseph Massino managed to cover up Vitale's previous work as a corrections officer.

The Italo-Canadian Cazzetta was not a member of the Mafia, but he did have a close relationship with the Montreal Mafia, and as a result of his Mafia ties, the Hells Angels were unwilling to challenge the Rock Machine as long as he was leader. Cazzetta has often been described as controlling all of the organized crime in Montreal that was not controlled by the Mafia in the late 1980s-early 1990s. The Hells Angels had first entered Quebec in 1977, but the group was badly weakened by the Lennoxville massacre on 24 March 1985 when five members of the Angels' chapter in Laval were shot by their colleagues. Afterward, the vacuum was filled by a number of Montreal-based organized crime groups such as the Rock Machine, and it was not until the early 1990s that the Angels became a major force in Montreal organized crime again.
