As for what exactly it will take to reverse these trends, the new study offers some ideas: "Meeting the needs of people living with HIV will require a combination of concentrating development assistance for HIV on these low-income countries, improving the efficiency of HIV programmes, increasing domestic financing, lowering the cost of treatment (including prices of ART drugs), and reducing future incidence through more concerted efforts," the authors write.

The Mexican health system is already overloaded by its own population, Janeway said, let alone the additional immigrants, asylum-seekers, and deportees from the US. It's difficult for immigrants to access health coverage; Tijuana General Hospital always has a hard time meeting the needs of the people who show up; and there aren't enough respirators for those who may need it if the coronavirus spreads as it's expected to.

A 2013 Institute of Medicine evaluation sums it up well: PEPFAR's efforts have saved and improved the lives of millions of people by supporting HIV prevention, care, and treatment services; meeting the needs of children affected by the epidemic; building capacity; strengthening systems; engaging with partner country governments and other stakeholders; increasing knowledge about the epidemic in partner countries; and ensuring that attention be paid to vulnerable populations in the response to HIV.
