The game is popular in many communities throughout Atlantic Canada (New Brunswick, Newfoundland, Nova Scotia, Prince Edward Island) as well as the Gaspé Coast in Québec. Forty- fives is also played in parts of Massachusetts and southern New Hampshire in New England, United States, as well as in the South Island of New Zealand. There are several regional variations in game play.

In the 1920s, French Canadian economic migrants who moved south into Massachusetts and New Hampshire in New England introduced the game, where it continues to be popular, sometimes under the French name quarante-cinq. In this region the game is most popular in southern New Hampshire and the Merrimack Valley of northeastern Massachusetts. Forty fives tournaments are becoming increasingly popular there. For example, the New England Academy of Forty-Fives holds occasional tournaments in Plaistow, New Hampshire, and Methuen, located in the Merrimack Valley, recently held a Forty-Fives tournament.

The Forty-Fives were both cowardly and brutal, and their modus operandi was to attack unprotected villages and slaughter the women and children. One night, a group of Cheyenne warriors managed to ambush the Forty- Fives, appearing behind their sentries and disappearing like ghosts, after killing all of them. The Forty-Fives' leader, horribly mutilated, pronounced a curse on the land shortly before he died. When a visiting scientist is found murdered, the Sheriff develops his own theory of the case: Medicine Creek and a neighboring town, both of which are dying economically, were competing for a contract from Kansas State University, to plant an experimental field of genetically-engineered corn for ethanol production.

Once all hands have been dealt the "bidding" begins with the player to the dealer's left, and proceeds in turn around the table.

Martin Francis Lamble (28 August 1949 - 12 May 1969) was the drummer for British folk rock band, Fairport Convention, from just after their formation in 1967, until his death in the band's van crash in 1969. He joined the band after attending their first gig and convincing them that he could do a better job than their current drummer, Shaun Frater. The eldest of three brothers, Martin was educated at Priestmead primary school, Kenton, and later at University College School, Hampstead. Pendergast surprises the police when he notices that the arrows are genuine 19th century Cheyenne artifacts, and moreover are almost impossibly well-preserved. Pendergast shares some of these details with the editor of the town newspaper, in exchange for an overview of the town and its population. Based on his information, the editor produces a somewhat sensationalist news story, associating the murder with a local town legend, "The Curse of the Forty-Fives." But Pendergast upsets both the Sheriff and the editor with his opinion that the killer is local to Medicine Creek, for the simple reason that the town is so isolated that no one could have entered the area from outside without being seen.
