"This week’s cliffhanger was very predictable with Tyler possessed. However, Paul did a wonderful job directing the episode. And it was a pleasant surprise seeing Kol and Vicki pop up on the ‘other side.’" Carrie Raisler of The A.V. Club gave the episode a B- rating and she compares the true love curse to the Sun and Moon curse, both of them turning out to be fake.

Jen from TV Overmind rated the episode with 7/10 saying that the episode left her feeling a little uneasy about the second half of the season and the next week's midseason finale.

Enzo has been lurking around in the shadows of most of this season’s stories, but he finally got his very own showcase here, and it was a doozy." Stephanie Flasher of TV After Dark gave the episode a B rating saying that the episode was very "Denzo-centric". "In summary, [Man on Fire] was an acceptable episode of the hit supernatural show. The writers did a great job showcasing Michael Malarkey’s talents as an actor in this episode. The direction was great too, allowing the audience see the varying sides of Enzo’s character.

Arriving at Los Angeles International Airport, Dr. Richard Thorndyke has several odd encounters (such as a flasher impersonating a police officer, and a passing bus with a full orchestra playing inside it). He is taken by his camera-happy driver, Brophy, to the Psycho-Neurotic Institute for the Very, Very Nervous, where he has been hired as a replacement for Dr. Ashley, who died mysteriously–Brophy suspects foul play. Upon his arrival, Thorndyke is greeted by the staff, Dr. Charles Montague, Dr. Philip Wentworth, and Nurse Charlotte Diesel. Thorndyke hears strange noises coming from Nurse Diesel's room and he and Brophy go to investigate.
