Tommy asked for replacement footwear for Christmas, but instead got an eyepatch with a Christmas tree on it, since he idolized pirates. Driven insane by this, Tommy began murdering his tormentors, though most consider him an urban legend.

The World's Greatest Super Friends episode "Universe of Evil" features Superman encountering evil versions of the rest of the team from an alternate universe, called the "Super Enemies" (he temporarily swapped places with his own evil counterpart, who wrought havoc and almost defeated the rest of the Super Friends until they swapped back just in time) when trying to stop Mount Vesuvius from erupting (which their Superman was causing). This universe's version of the Hall of Justice is called the Hall of Evil, and a demonic-looking face is on the outside of the building. The Super Enemies themselves appear almost identical to the Super Friends, although their version of Aquaman has an eyepatch, Batman's costume is red rather than blue, and Robin has a moustache.

As the castle collapses, Tajima is confronted by Jubei, who has covered his body with Buddhist prayer symbols and chastises his father for obsessing only on his swordplay to the point of coming back from hell to fight his own son. Shiro appears and offers to let Jubei join him; Jubei declines and vows to set his father and Kirimaru's souls at peace. Suicide Squad #27, 1989 Psycho-Pirate does not appear again until 1995 when he became part of the Underworld Unleashed crossover event. In this story Psycho-Pirate, along with several other villains, sells his soul to the demon Neron in exchange for more power. The process results in a costume change for the character as well, with Hayden now wearing a black leather jacket and with the metal of his mask morphing into an eyepatch and replacing about half of his brain. Once again sane, and making no allusion to the events of Crisis on Infinite Earths and the existence of the Multiverse, Psycho-Pirate goes on to fight the Chase Lawler version of Manhunter before being arrested and sent to jail.

In the episode "A Dark Knight: The Sinking Ship, The Grand Applause", the Headhunter gets out of the hospital, where he now wears an eyepatch and meets Sofia Falcone at the time when Victor Zsasz brings him to raid Arkham Asylum to target Cobblepot, only for him to be sprung from Arkham by Edward Nygma. The two of them encounter Gordon and Harvey Bullock on the streets with Cobblepot, which resulted in a gunfight where Cobblepot got away with Leslie Thompkins. When Sofia Falcone brought Zsasz, the Headhunter and some Falcone crime family operatives to Spa Bo'sh Sumka in order to target Arthur Penn, Zsasz and the Headhunter pursue Bullock and Penn. While the two of them got away in Leslie's car, Zsasz and the Headhunter went out for smoothies when they saw the police cars arriving.

The next to die is awkward nerd Dorcas Cunningham, who Tommy sneaks up on with a running chainsaw, only being noticed when he steps on a twig. After the sultry Abby Honeydew is stabbed while looking for their missing friends, Ernie and Danny make plans to try and summon Tommy's previous victims with a Ouija board, but Ernie is killed with a screwdriver while traversing a cemetery. Danny fights Tommy, and stabs him in the head with a rail spike, though Tommy recovers from this, blasts Danny with a shotgun, and takes one of his shoes. Five years later, Tommy (who still has the spike protruding from his head) is shown to have married the woman he had sex with, and the two have a son, who Tommy gives his eyepatch to as a Christmas present.

Although eyepatches have since become stereotypically associated with pirates, the source is unclear, and there is no historical evidence to suggest that their use was for any other reason than protecting and concealing the eye socket after the loss of an eye. Most historical depictions of seamen with eye patches are of ex-sailors, rather, than pirates. More recent medical texts have often referred to the eye patch as a "pirate's patch" and writing in the Minnesota Academy of Sciences Journal in 1934, Charles Sheard of the Mayo foundation, pointed out that by "wearing a patch (The pirate's patch) over one eye, it will keep the covered eye in a state of readiness and adaptation for night vision".

They consist of: Shamkid "the leader" who can transform into a cat and can foretell a person's actions after having a piece of their hair; Sam "the veteran" who sports a ponytail and an eyepatch, and wields a katana; Jasmine "the looks"who is dressed as a jiangshi, speaks Chinese, and wields a claw weapon; Roy "the brain" who wears a striped shirt, handles navigation for their team airship, and whose special ability includes falling asleep to enable a "chat room" that allows the team to communicate with each other telepathically; and Rando "the brawn" a mummy who was able to capture Fuyumi inside his body.

Kano originally had a large helmet with two red eyes, a concept that was dropped in favor of a smaller metal piece over his right eye. The eye piece was at first envisioned as a simple eyepatch, but then it was turned into a metal plate inspired by Arnold Schwarzenegger's character in The Terminator.. In his Mortal Kombat: Deception biography card, Mortal Kombat co-creator Ed Boon described how Kano's bionic eye in the first game was created by cutting out a piece of a plastic mask and painting it a silver color, then attaching it with spirit gum to actor Richard Divizio's face, with the infrared eye added digitally.

Samson is a heroic barbarian adventurer endowed with superhuman size and strength living in a future where nuclear war has bombed the world back into a second Stone Age. Leaving his home tribe after the death of his mother, he loses an eye to a ferocious hybrid creature called a liobear (a mixture of a lion and a bear), which the blond giant manages to kill in unarmed combat, later skinning and wearing its red pelt caveman-style throughout most of his adventures and using a strip of its hide as an eyepatch. Badly wounded in the battle, he is found and nursed back to health by the beautiful golden-haired Sharmaine, whose father Mindor is a bespectacled, white-coated scientist who extrapolates forgotten 20th-century knowledge from ancient artifacts he discovers in the ruins of N'Yark (New York). In gratitude, Samson decides to join in their quest to restore the benefits of civilization to mankind and protect them from the bizarre mutant beasts and savage tribes that dwell among the once famous landmarks of the rubble-strewn, jungle-choked city.

Ostensibly an extremely low-budget children's television programme featuring two pirate crows: the titular Histor (who concealed a multicoloured spinning eye beneath his eyepatch) and his hapless first mate Pliny Harris. Histor's ability to transport himself and Pliny through time ("as the crow flies") to view past events would be used to satirise current affairs, and the script would be peppered with deliberately weak but dense nautical- and bird-related multiple puns, which would increase in volume and weakness as the series progressed. (For example, Pliny would say 'Egg feather bird oeuf tit' in place of 'I've never heard of it', or 'Feather me wingers' in place of 'Shiver me timbers'.) Pliny's idiocy drives Histor to insanity and, eventually, he murders him by stuffing him with eggs until he bursts, as he keeps using the word "egg" so it has no connection or relation to the context of what Histor was saying, (only for Histor to be subsequently haunted by Pliny's equally pun-obsessed ghost). A running joke in this segment was that despite Pliny's apparent idiocy, he would occasionally counter Histor's right-wing views with extraordinarily eloquent and well constructed left-wing arguments.
