Men near the pike were able to escape, but most of the others sought refuge in security corridors or at the bottom of the pikes. Emergency exits were improperly marked, hindering the escape of many until too late. Men began to succumb to carbon monoxide poisoning because of the smoke, which left them unconscious. Some 355 miners died; another 747 were injured.
A fire at the Unitop store killed 25 people on Christmas day. The fire started from firecrackers and pyrotechnics that were ignited near the entrance door. The store did not have a permit to sell firecrackers and emergency exits were locked. The charred remains of the victims were found inside a bathroom, where they tried to escape from the blaze.

All commercial cinemas (defined as any business which admitted members of the public to see films in exchange for payment) had to comply with these regulations. 51st Street on the IRT Lexington Avenue Line, opened on July 17, 1918, is a local station with two local tracks and two side platforms. The two express tracks, used by the and trains during daytime hours, pass through a lower level and are not visible from the platforms. Both platforms have emergency exits from the lower level express tracks. The station features modern beige bricks over the original tiles, but the standard IRT-style mosaics remain intact.

There also were bars in front of the windows and there were too few emergency exits, all of which contributed to the high number of injuries. The first report reached the Amsterdam ambulance service CPA at 00:38 local time on 1 January. The first fire engine arrived at 00:46 CET. The Dutch Red Cross reported that the alarm was raised shortly after 00:30 local time and that it took 15 minutes for the first ambulances and medical teams to arrive.
However, three of the eight emergency exits could not be opened and several escape slides did not inflate (though with the fuselage sitting on the ground this was not a great problem). The tower controller, aware shortly after touchdown that something was amiss, activated the emergency alarm. However, the emergency bell did not ring. According to the authorities, other reasons for the high death toll include the lack of emergency exits (the only access in and out of the building was the front door) and the fact that the number of people inside exceeded the maximum capacity by hundreds. About 90% of the victims succumbed to smoke inhalation. Many people died as they either tried to hide in bathrooms or, in panic mistook them for exits. At least 180 bodies were removed from the bathrooms.
BP-1102 was used for water egress trainer for all Apollo flights, including by the crew of Apollo 11, the first lunar landing mission. It was also adapted for mock-up interior components and used by astronauts to practice routine and emergency exits from the spacecraft. It was then modified again where the interior was set up to be configured either as Apollo/Soyuz or a proposed five-person Skylab Rescue vehicle. With these two conversions, astronauts could train for those special missions.

Snell-Hitchcock Amos Jerome Snell Hall and Charles Hitchcock Hall, more commonly known as Snell–Hitchcock (colloquially Snitchcock), make up a residence hall at the University of Chicago. The dorm is on the northwest corner of the University's main quadrangles at the corner of 57th St. and Ellis Avenue. It is connected via emergency exits to Searle Chemistry Laboratory. Built in 1892 (Snell) and 1901 (Hitchcock), they are the oldest residence halls still in use as such on the university's campus.

This design was built into the original plans, in which the road administration anticipated building the crossing as a full motorway in two phases. The practice is actually quite common in the absence of fires, as well. Some skyscrapers have stairwells with standard emergency exit signs on each door, which then lock upon closing. Users of these stairwells are trapped, whether they know or do not know that the only door that opens from the inside is the one on the ground floor.

There is an agreement between AIS Manila and Medical Center Manila to ensure that students receive immediate and excellent medical treatment in the event of an emergency. AIS Manila has comprehensive emergency policies for fire, earthquake and other potentialities. Two bands were scheduled to perform that night ("Pimenta e seus comparsas" and "Gurizada Fandangueira"). In the early morning hours of 27 January 2013, a fire broke out while students from the Federal University of Santa Maria (UFSM) were holding a freshers' ball. A stampede occurred following the fire, and a lack of exit signs and emergency exits allegedly contributed to the deaths.
Audience members waited for the fire to be extinguished by theatre personnel, wasting the precious minutes they needed to escape safely. The stage and auditorium were located on the 2nd floor and the few emergency exits available were either unmarked or blocked. Two fire escapes were available but were only accessible through latched windows whose sills were located 3 feet above the floor. Of the approximately 400 men, women, and children either in attendance or associated with the performance of the play, 171 perished in various ways as they tried to escape the conflagration.
Portsmouth eventually won the Division Two championship, with Cardiff City finishing as runners-up. Following the 15 April 1989 Hillsborough Disaster, Portsmouth F.C. removed the perimeter fences from Fratton Park for the 1989–90 season, except at the Milton End to separate away supporters, although these too were removed for the following 1990–91 season after the Taylor Report was published in January 1990. However. the Milton End moat and wall have been retained to present day, with additional emergency exits retrofitted in. In the summer of 1996, Fratton Park became an all-seater stadium.

The air pressure release is a handle beneath the rectangular center window that is similar in appearance to a car door handle. If a train is stopped on an open beam, then guests evacuate through emergency exits located in the roof of the train. Guests open roof hatches by first removing decorative plastic from the ceiling above a bulkhead footstool and then by lifting open a hinged hatch that will flip across the bulkhead dividing two train cars. Kitchener Market is a stop on the Ion rapid transit system in the Region of Waterloo in Ontario, Canada. It is located in the Cedar Hill neighbourhood at the intersection of Charles and Cedar streets, about south of its namesake, the Kitchener Farmer's Market. The station initially had a working name of Cedar, and it opened in 2019. The primary access to the platform is from the crosswalk at Cedar Street; secondary accesses at the east end of the platform, crossing either side of Charles Street, are marked as emergency exits only.
However, these moves were not globally copied for some time. For example, in the United States, 146 factory workers died in the Triangle Shirtwaist Factory fire in 1911 when they were stopped by locked exits, and 492 people died in the Cocoanut Grove fire in a Boston nightclub in 1942. This led to regulations requiring that exits of large buildings open outward, and that enough emergency exits be provided to accommodate the building's capacity. Similar disasters around the world also resulted in public fury and calls for changes to emergency regulations and enforcement.

It is believed that unidentified members of the Jalisco drug cartel were angered at the bar owner's refusal to pay extortion demands, and burst into the bar at gunpoint. The unknown assailants then locked the doors and other emergency exits of the club and then doused the building with gasoline and set it on fire. Prior to the attack the owner was kidnapped by the same group of individuals. Early reports of the attacks claimed that the fire had been started by homemade bombs, although it was later recanted.

Following the study, plans were developed to repair the structure block-by-block. In the MTA's 2015–2019 Capital Program $180 million was provided to replace the structure under Park Avenue between 48th Street and 49th Street in addition to other locations. However, the July 2017 amendment to the program reduced the project to $67 million, moving the first stage of construction to the next capital program. The program also included $1 million in funding to study improvements to improve fire safety in the Park Avenue Tunnel, including the installation of improved emergency lighting and wayfinding signage directing to emergency exits.
Shortly after the collision, governor Wallace Wilkinson ordered his cabinet to review the state's drunk driving laws and bus safety regulations. At a news conference on May 20, 1988, Wilkinson announced stricter enforcement of drunk driving by the state, including police sobriety checkpoints and more frequent inspections by state Alcoholic Beverage Control. Pakistan quickly sent in the Pakistan Army's Special Services Group (SSG) commandos and Pakistan Rangers were put on high alert. The 17-hour long hijacking came to an end when the hijackers opened fire on the passengers at 21:30 Pakistan Standard Time, but soon ran out of ammunition, resulting in some passengers fleeing the aircraft through the aircraft's emergency exits. The SSG responded by storming the aircraft and seizing the hijackers. The SSG commando unit was headed by Brigadier Tariq Mehmood and the Shaheen Company of the SSG's 1st Commando Battalion carried out the operation.

Malacañang reiterated the importance of President Rodrigo Duterte's campaign against criminality as a "centerpiece program" of the administration. President Duterte called the gunman "crazy", ruling out any links to ISIL saying the group was more "brutal and cruel". While speaking to reporters in Subic Bay, Duterte apologized for the incident and questioned Resorts World Manila on the layout of their emergency exits.
To allow for the connection to be built, a 900-foot long tunnel would be built under 42nd Street from the west side of Third Avenue to Second Avenue. The library was reportedly bombarded with phone calls. McDonald was eventually escorted away by police and he said the group blocked emergency exits. In February 2019, using his Facebook account, Robinson wrote "I guess it's ok to rape white women then?" next to a Rape Crisis flyer about specialist services for ethnic minority victims, resulting in hundreds of racist and abusive phone calls to the centre from Robinson's supporters. The centre, which was providing support for rape victims of all ethnic backgrounds, condemned Robinson's post for "disrupting much-needed service provision for victims and survivors of sexual violence and abuse of all ethnicities and backgrounds".

Witnesses reported that many workers had been unable to escape through the narrow exits of the building. Twelve of the victims died leaping from windows to escape the flames, some of whom would die of other injuries after they had been taken to area hospitals. After touchdown, the aircraft did not stop before the end of the runway, but continued on for another until it slid into the Etobicoke Creek ravine at a speed of , on the western edge of the airport near the interchange of Dixie Road and Highway 401. After the aircraft had stopped, the crew saw fire outside and began evacuation. When the emergency exits were opened, one of the right middle exit slides (R3) deflated after being punctured by debris from the aircraft, while one of the left slides (L2) failed to deploy at all for unknown reasons. Neerja Bhanot, Ashoka Chakra (7 September 1963 – 5 September 1986) was an Indian head purser who died while saving passengers on Pan Am Flight 73 which had been hijacked by terrorists during a stopover in Karachi, Pakistan, on 5 September 1986, just two days before her 23rd birthday. Posthumously, she became the youngest recipient of India's highest peacetime gallantry, the Ashok Chakra Award, as well as several other accolades from the government of Pakistan and the United States. She was shot while helping passengers escape through the emergency exits. Her life and heroism inspired the biopic Neerja, which was released in 2016 and was directed by Ram Madhvani starring Sonam Kapoor.
Fire exit in London. This was not originally a fire exit, as there is an engraving of the name of the building, but is now only used as a fire exit. Following the events of the Victoria Hall disaster in Sunderland, England in 1883 in which more than 180 children died because a door had been bolted at the bottom of a stairwell, the British government began legal moves to enforce minimum standards for building safety. This slowly led to the legal requirement that venues must have a minimum numbers of outward opening emergency exits as well as locks which could be opened from the inside.

Both of the rebuilt coaches remained in service with McCafferty's for more than a decade.
President Thabo Mbeki held a minute's silence for the dead at his May Day speech the following day, and the dead were buried together in a mass funeral at West End (Wes einde) cemetery in Kimberley. The cause of the crash was believed to be negligence on the driver's part, in his failure to prepare a route through a hazardous area, which was compounded by a lack of signs and gates on the dam's access roads. The bus too was an old model, and did not possess efficient emergency exits or safety equipment, as well as suffering from poor brakes, which may have contributed to the disaster. The dam was renamed the Sol Plaatje Dam on 1 April 2005.

In 2011, through a campaign to promote the island of Aruba, Tiara Air had been branded as the official airline of Aruba. Because of this, most of Tiara Air's fleet had received the official logo of Aruba applied on their fuselage. The modified livery on the Shorts 360 features yellow and blue stripes on the lower part of the fuselage with a grey belly, the logo's signature T, and the crown featured on the vertical stabilizer and Tiara Air written above the forward emergency exits along with the star of the Aruban flag. The wheel pants are also grey of color and feature the aircraft type on it written as SD-360 on the aircraft.
The British Air Ministrys Inspector of Accidents reported that the accident had been caused by icing in the carburetters of all four engines. This caused a full loss of power in the inboard engines and partial loss in the outer; the commander of the Cavalier had reported icing problems prior to ditching. The inspector recommended that extra heating of carburetters and of the incoming air be provided and that a temperature indicator be installed. He also advised that passengers should be instructed in the fastening of lifebelts and the location of emergency exits and recommended the provision of extra life-saving equipment like rafts and pyrotechnic signals and that passengers should fasten safety belts at take-off and alighting.

One of the gay centers of culture and amusement in Mexico is the Zona Rosa, a series of streets in Colonia Juárez in Mexico City. Since mid-2007, the government of the Federal District and Cuauhtémoc, D.F.—in whose territory the Zona Rosa is found—have placed operatives in some seedy nightclubs of the Zona Rosa, with the purpose of freeing this tourist zone of problems such as illegal drug trade and prostitution, as well as reducing the incidence of crimes such as theft. Other targets of the program are those sites of cohabitation that lack safety measures for the users—mainly emergency exits. LGBT groups have denounced the action as a form of homophobia.
Additionally, the NTSB stated: "The tires on the aircraft may have been operated in the overdeflected condition, since the average inflation pressure was less than the optimum pressure for maximum gross weight." The NTSB made recommendations to the Federal Aviation Administration (FAA), including that the FAA prohibit mounting on the same axle different models of tires which had different load- bearing characteristics and also that greater load-bearing characteristics be required in tires manufactured in the future. The NTSB also issued a series of recommendations regarding improvements to aircraft evacuation safety, including development of more durable and fire-resistant slides, and the placement of evacuation ropes at emergency exits for use in the event of slide failure.
The resulting blaze killed four others due to smoke inhalation, because the electrical closet was placed next to the main hallway between the offices and the emergency exits, blocking their escape due to the extreme heat and dense smoke.
