The Carnate derailment occurred in Lombardy, Italy on 19 August 2020 when a passenger train ran away from Paderno-Robbiate station [it] and was derailed at Carnate-Usmate station.

Festers are the reincarnation of slave traders who ran their slave ship aground on Carnate, and left their slaves tied up in the hold to be eaten by rats, rather than freeing them. Infernas are the trapped spirits of three children who accused eleven innocent people of being witches during the time of a Puritan settlement on the island. The eleven people were burnt alive.

Horace P. Gauge (John Armstrong) was an inmate who was executed in the electric chair for murdering his wife during a conjugal visit. Full of guilt for his actions, he claims Carnate drove him to it, and wants to help Torque realize his decency. He appears in the form of electricity. They teach Torque how to unleash the anger inside himself and transform into a powerful monster, although the transformation is only in Torque's mind.

Bad ending After Torque defeats the creature, he remembers the murders. Realising that he was the one who killed his family; he smashed his wife's head in, drowned his youngest son, and threw his eldest son out of a window. When the coast guard arrives, Torque transforms into his monster state and kills him, before running back into the woods of Carnate. Neutral ending After Torque defeats the creature, he remembers the murders.

Slayers represent decapitation, Marksmen represent death by firing squad, Mainliners represent lethal injection, Noosemen represent hanging, and Burrowers represent burial alive.

The station was officially opened on 17 August 1840, as the terminus of the Milan–Monza railway, which was the first railway built in Lombardy and the second in Italy, after the Naples–Portici railway. Operations commenced the following day, 18 August 1840. In July 1849, that line was extended, to Camnago-Lentate, on its way to becoming the Milan–Chiasso railway. The train was reported to have run away from Paderno d'Adda without crew, and with a passenger detected after the derailment. The train was to operate a Paderno d'Adda-Milan service about 25 minutes after the start of the runaway from Paderno d'Adda, which took place about 15 minutes after its arrival from Milan; it included a Class E.464 locomotive (in the rear position), six carriages and a control car. After travelling for , the train was diverted into a siding at Carnate-Usmate station. The control car and the first three carriages of the train derailed.

The game tells the story of Torque, a prisoner on death row for murdering his ex-wife and two children, a crime of which he claims to be innocent, saying he blacked out and cannot remember what happened. Shortly after he arrives in Abbot State Penitentiary on Carnate Island, off the coast of Maryland, an earthquake hits the island, resulting in the prison being attacked by unusual supernatural threats. After being freed, Torque attempts to fight his way to freedom while confronting his own personal demons, all while attempting to remember what really happened to his family.

Shortly after entering his cell, an earthquake rocks the prison, and moments later, all of the inmates on death row except Torque are killed by strange creatures. One of the creatures opens Torque's cell, and he sets out to escape.

The railway from Lecco to Bergamo was built by the Società delle Strade Ferrate della Lombardia e dell'Italia Centrale (Lombardy and Central Italy Railway Company) and opened on 4 November 1863. The section from Lecco to Calolziocorte has also been used by the Lecco–Milan railway since 1873. The Bergamo–Ponte San Pietro section has also used been by the Seregno-Bergamo line since the completion of the Carnate–Ponte San Pietro section in 1889. In 2015, the Province of Bergamo, the region of Lombardy and some local parliamentarians requested and obtained government funding for the doubling of the section between Ponte San Pietro and Montello, the building of the new Bergamo Ospedale station at the Papa Giovanni XXIII hospital and a suburban service between the two Bergamo stations.
