Gertrude meets Maddie, a denizen of Fairyland and her self-proclaimed "biggest fan", who idolises Gertude for the violence and death she has spread. Although initially happy to take Maddie on as a protege, Gertrude quickly becomes sick of her, and realizes that she does not like the person she has become. She murders Maddie and resolves to become a better person. Despite her best intentions, Gertrude repeatedly fails to do good, leaving just as much death and destruction in her wake.

Vijay, with the help of his friend Abdul Sattar (Johnny Walker) escapes from the mental asylum and reaches the memorial service, where he denounces this corrupt and materialistic world. Seeing that Vijay is alive, his friend and brothers side with a rival publisher for more money and declare that this is Vijay. At a function to honour him, Vijay becomes sick of all the hypocrisy in the world around him and declares he is not Vijay. He then leaves with Gulabo to start a new life.

The Boyce and Alan have a complex relationship, apparently hating each other but displaying signs of obvious affections for each other nonetheless. In the second series, Alan wins a caption competition on the Consultant Radiologists International website, an organisation of which Alan claims to be an "esteemed member". Joanna becomes sick of his cheerfulness, so she tries to scare him using her dwarf cousin, but Alan is so shocked that he believes he is hallucinating, and beats the man to death with a stuffed heron. Alan and Joanna flee the hospital, assisted by Boyce, and they steal a camper van which, unbeknownst to them, contains Joanna's son, Dr Martin Dear.

He justifies his cause by using the example of a fish — a fish in the sea cannot belong to anyone, but once it has been caught, it is the property of the fisherman. Trachalio, showing his quick-wit, states that if he is correct, the trunk is as much his as it is Gripus'. The argument becomes increasingly obscure as Trachalio demands in which way a trunk can be treated in the same way as a fish, to which Gripus states that there is such thing as a trunk- fish. Trachalio becomes sick of the argument, and instead suggests that they seek an arbitrator to make a decision — characteristic of Gripus, he suggests a tug of war — an idea much scorned by Trachalio.
