In a local hospital, Joe has visions of a beautiful veiled woman with mystical powers. Joe's leg later becomes infected and he finds himself in a hospital in Hong Kong, and finds the woman from his dreams earlier named Sheila (Emily Chu) magically healing his legs and granting him superhuman powers such as the ability to make large leaps through the air. The risk of transmission from mother to baby is highest if the mother becomes infected around the time of delivery (30% to 60%), since insufficient time will have occurred for the generation and transfer of protective maternal antibodies before the birth of the child. In contrast, the risk falls to 3% if the infection is recurrent, and is 1–3% if the woman is seropositive for both HSV-1 and HSV-2, and is less than 1% if no lesions are visible. Women seropositive for only one type of HSV are only half as likely to transmit HSV as infected seronegative mothers. To prevent neonatal infections, seronegative women are recommended to avoid unprotected oral-genital contact with an HSV-1-seropositive partner and conventional sex with a partner having a genital infection during the last trimester of pregnancy.

The group begins to suspect that Packer is really only interested in following the trappers to find his horse, and Bell's temper begins to shorten as his wound from the bear trap becomes infected and develops gangrene. They soldier on until they encounter a foreboding "Cyclops", a tall mountain man with a wounded, pus-spurting eye, while unwittingly trying to steal one of his sheep for sustenance. The Cyclops recalls how a Union soldier shot out his eye in the Civil War, and becomes enraged when he realizes Packer's men are not “Southern boys” after they cannot finish the lyrics to "Dixie", causing them to flee. They escape and the badly frostbitten Swan tries to cheer everybody up with a song about building a snowman, only for Miller to destroy Swan's snowman with a pickaxe ("Let's Build a Snowman").

Irina Derevko recites a similar prophecy at the finale of season 4 that Rambaldi wrote: :When blood-red horses wander the streets and angels fall from the sky, the Chosen One and the Passenger will clash . . . and only one will survive. When Elena Derevko put in motion her scheme to destroy civilization using the contaminated water in conjunction with a giant Mueller device, the team spotted a horse that under the light of the device appeared blood red (which prompted Irina to recite the prophecy); Nadia passed a statue of an angel which had fallen from a building right before she was captured and later becomes infected with the contaminated water. As predicted by Rambaldi, "The Chosen One" and "The Passenger" clashed over the Mueller device, and Sydney would have been killed if not for Arvin Sloane's intervention when he was forced to shoot Nadia when she began strangling Sydney.

Meanwhile, Zombie Wolverine kills Shang-Chi, Elektra, Sunfire and Iron Fist until Earth-Z's Wolverine appears, engaging in a fight. With the intervention of Kitty Pryde and Zombie Spider-Man, Wolverine manages to kill his zombie counterpart, his zombified victims and lets Zombie Spider-Man to get a sample of his blood to develop a cure, and while he becomes infected through unknown reasons, he resists the hunger and doesn't spread the contagion. The fourth issue follows the return of Earth-Z's Hulk to the Blue Area of the Moon years after Zombie Wolverine's attack on Japan and following his banishment to Sakaar by the Illuminati. Upon arriving along with the Warbound, Hulk discovers Zombie Giant-Man and the zombified Inhumans, which leads to the deaths of all the Warbound members and with the former infecting him before he makes his way back to Earth.
