Animality finishing moves were also removed, ironically keeping the Mercy move, which originally served as a requirement for Animalities in MK3. On the other hand, Brutalities were introduced; a finishing move in which the player attacks their opponent with a series of kicks and punches which result in the victim exploding. At the same time, some changes were actually improvements over the arcade version. Rain and Noob Saibot are made into playable characters for the first time.

It also introduces new types of the finishing moves, including Animalities. Other features new to the series were combos, predefined sequences used to perform a series of consecutive attacks. A "Run" button was also added, allowing players to briefly dash toward the opponent, as were "Kombat Kodes", an unlockable content system using various symbols that can be entered before two-player matches to achieve certain effects. Some characters from the previous games returned and new characters were introduced into the series.

Another leftover from MK3 that was not totally deleted was code data for the 'Endurance Mode', which is still accessible through a Game Genie code. There were, however, several differences. Unlike the SNES version, the Genesis version features more stages: with the addition of the five new ones, it also features six of the original ones from MK3, including the Subway, Bank, Rooftop, Soul Chamber, The Temple, and The Pit 3. There are several additional cuts regarding special and finishing moves: both Animalities and Mercy were removed; Rain and Noob were given a Brutality, but no other finishing moves; Human Smoke shares Scorpion's combos, rather than having unique ones; in Stryker's Friendship, the running characters are replaced by dogs.

Gameplay screenshot Tattoo Assassins was developed to compete with the popular Midway fighting game franchise Mortal Kombat. The project was spearheaded by Joe Kaminkow (leader of Data East Pinball, a subsidiary of Data East Japan) and Bob Gale (screenwriter for Back to the Future) and put into development at Data East Pinball for a planned arcade release. Using the same style of digitized graphics as Mortal Kombat, Tattoo Assassins featured real- life actors fighting each other with violent moves and combos. Most notable is that the game claims to feature 2196 finishing moves, including some nudity- based finishers (only a rumor in Mortal Kombat) and animal-based finishers (before they were featured in Mortal Kombat 3 as "Animalities").

The game was originally conceived as an HD remake of the early Mortal Kombat games, featuring remade character sprites, new backgrounds and new user interface system, before the scope of the project was changed. Known as Mortal Kombat HD Arcade Kollection, it was supposed to be made by Other Ocean Interactive, the studio that has created Ultimate Mortal Kombat for the Nintendo DS. This canceled project was never even officially announced before it was canceled, and a simple compilation of original games was released instead of it, also created by Other Ocean. Graphical assets, such as Zbrush models for the Animalities and Mileena's unmasked face have been since leaked by the artist Jeremy Kohar in 2013. Previously, more concept art from the project was revealed by the artist John Montenegro.

While many games have been subject to urban legends about secret features and unlockable content, these kinds of myths were particularly rampant among the dedicated fan community of Mortal Kombat in connection with Mortal Kombat II. According to GameSpy, "the [arcade gaming] community was abuzz about myriad secrets both true and false." The game's creators did little to dispel those rumors that included supposed "Nudality" or "Sexuality" finishing moves for Kitana and Mileena, Shang Tsung's ability to transform into Kano and Goro, a chance to fight Sonya after defeating Jade in a specific way, and "Hornbuckle" being featured as an additional secret character. Some of them were eventually implemented in subsequent MK games. Among these rumors to be adapted later were the Animalities (used in Mortal Kombat 3 and its updates) and an ability to throw an opponent into the mouth of a tree in the Living Forest stage (first used in Mortal Kombat: Shaolin Monks).
