A prolonged stimulation either through foreplay (kissing, petting and direct stimulation of erogenous zones before penetration during intercourse) or stroking (during masturbation) leads to an adequate amount of arousal and production of pre-ejaculatory fluid.

Many women felt inadequate when they could not achieve orgasm via vaginal intercourse alone, involving little or no clitoral stimulation, as Freud's theory made penile- vaginal intercourse the central component to women's sexual satisfaction. The first major national surveys of sexual behavior were the Kinsey Reports. Alfred Kinsey was the first researcher to harshly criticize Freud's ideas about female sexuality and orgasm when, through his interviews with thousands of women, Kinsey found that most of the women he surveyed could not have vaginal orgasms. He "criticized Freud and other theorists for projecting male constructs of sexuality onto women" and "viewed the clitoris as the main center of sexual response" and the vagina as "relatively unimportant" for sexual satisfaction, relaying that "few women inserted fingers or objects into their vaginas when they masturbated".

O'Connell et al., who performed dissections on the female genitals of cadavers and used photography to map the structure of nerves in the clitoris, were already aware that the clitoris is more than just its glans and asserted in 1998 that there is more erectile tissue associated with the clitoris than is generally described in anatomical textbooks. They concluded that some females have more extensive clitoral tissues and nerves than others, especially having observed this in young cadavers as compared to elderly ones, and therefore whereas the majority of females can only achieve orgasm by direct stimulation of the external parts of the clitoris, the stimulation of the more generalized tissues of the clitoris via intercourse may be sufficient for others. French researchers Odile Buisson and Pierre Foldès reported similar findings to that of O'Connell's.

The mass of smooth muscle and glandular-duct tissue in the nipple and areola block the development of normal dermal nerve networks which are present in other erogenous regions and the development of special end organs. The entire breast has a network of nerve endings, and it has the same number of nerve endings no matter how large the breast is, so that larger breasts may need more stimulation than smaller ones. Intense nipple stimulation may result in a surge in the production of oxytocin and prolactin which could have a significant effect on the individual's genitals, even to the point that some people of both sexes can achieve orgasm through nipple stimulation alone. Having the chest, breasts and nipples stimulated manually (hands), digitally (fingers, toes) and/or orally (mouth, lips, teeth, tongue) is a pleasurable experience for many people of both sexes.

Women who failed to achieve orgasm through penetrative, heterosexual intercourse were therefore labeled as dysfunctional or frigid by the professional community. In Koedt's view, this approach placed unfair blame on women for their lack of satisfaction during straight sex, inaccurately pathologized normal female sexual function, and caused many women to seek unnecessary psychoanalytic treatment for a nonexistent ailment rather than exploring techniques that would lead to a more pleasurable sexual experience. In support of her position, Koedt marshaled up-to-date research on the female anatomy and sexual response, including recent work by Alfred Kinsey and Masters and Johnson, to demonstrate that the clitoris, rather than the vagina, is the primary site of erotic stimulation. Koedt went on to argue that male chauvinism and the urge to maintain women in a subservient role were the primary driving force perpetuating misconceptions surrounding female sexuality.
