Researchers are required to obtain informed consent from potential participants before any research begins – this is a fundamental principle of medical research as laid out in the Declaration of Helsinki. Traditionally this has been done through a paper consent form which is accompanied by a subject information sheet that describes the risks and benefits of being involved in the research. Increasingly, this also outlines how an individual's data will be protected and their privacy maintained. This constitutes a formal agreement that specifies how a research participant’s data will be used in that particular study.

Structured documents generally focus on labeling things that can be used for a variety of processing purposes, not merely formatting. For example, explicit labeling of "chapter title" or "emphasis" is far more useful to systems for the visually impaired, than merely "Helvetica bold 24" or "italic". In the same way, meaningful labeling of the many items on a technical information sheet enables far better integration with databases, search systems, online catalogs, and so on. Structured documents generally support at least hierarchical structures, for example lists, not merely list items; sections, not merely section headings; and so on.

All auxiliary labels contain information important to proper medication adherence, or to avoid adverse drug reactions. There is not one guideline for how to best formulate auxiliary labels on a prescription, but the label should be as simple as possible and written in plain language. A 2016 study found that many patients consider side effects to be important information on a prescription package, and that auxiliary labels are a good tool to provide this information on the packaging itself (as opposed to a separate information sheet/handout). The same study found that patients associated the use of red as a highlight color with information regarding warnings, allergies, or side effects.

While health effects from extremely low frequency (ELF) electric and magnetic fields (0 to 300 Hz) generated by power lines, and radio/microwave frequencies (RF) (10 MHz - 300 GHz) emitted by radio antennas and wireless networks have been well studied, the intermediate range (IR) (300 Hz to 10 MHz) has been studied far less. Direct effects of low power radiofrequency electromagnetism on human health have been difficult to prove, and documented life-threatening effects from radiofrequency electromagnetic fields are limited to high power sources capable of causing significant thermal effects and medical devices such as pacemakers and other electronic implants.Electromagnetic fields & public health: Intermediate Frequencies (IF). Jones placed an advertisement in Jazz News (a Soho club information sheet) on 2 May 1962, inviting musicians to audition for a new R&B; group at the Bricklayer's Arms pub; pianist Ian Stewart was the first to respond. Later, singer Mick Jagger also joined this band; Jagger and his childhood friend Keith Richards had met Brian when he and Paul Jones were playing Elmore James' "Dust My Broom" with Korner's band at the Ealing Jazz Club. Jagger brought guitarist Richards to rehearsals; Richards then joined the band. Jones' and Stewart's acceptance of Richards and the Chuck Berry songs he wanted to play coincided with the departure of blues purists guitarist Geoff Bradford and singer Brian Knight, who had no tolerance for Chuck Berry.
After leaving the Queen Elizabeth's Foundation college and returning to live in Oldham, his interest in writing continued when he went to New Vale House, a day centre where he established a newsletter in 1984 which again was highly critical of the organisation. He was employed by the Greater Manchester Coalition of Disabled People in 1986 to be their first Information Worker, and he soon established its journal or magazine - Coalition - which gained a national reputation for the quality of its coverage of disabled people's politics. He also produced a regular Information Sheet with details of local events and contacts. The Coalition magazine was recognised in university-level textbooks and papers as being a key source of knowledge for disability studies.

However, treated wood may present certain hazards in some circumstances, such as during combustion or where loose wood dust particles or other fine toxic residues are generated, or where treated wood comes into direct contact with food and agriculture. Preservatives containing copper in the form of microscopic particles have recently been introduced to the market, usually with "micronized" or "micro" trade names and designations such as MCQ or MCA. The manufacturers represent that these products are safe and EPA has registered these products. The American Wood Protection Association (AWPA) recommends that all treated wood be accompanied by a Consumer Information Sheet (CIS), to communicate safe handling and disposal instructions, as well as potential health and environmental hazards of treated wood.

"Metabolic therapy", including administration of laetrile, was promoted for cancer patients by John Richardson in the San Francisco Bay Area in the 1970s, until his arrest for violating the California Cancer Law and revocation of his license by the California Board of Medical Quality Assurance. Given the frequent border conflicts between Eritrea and its neighbors, the army's numbers have always been maintained through conscription with the proclamation on the Eritrean National Service, for both men and women between the ages of 18 and 40, a policy officially introduced by President Afwerki in 1995. Thus Eritrea has remained in a state of mobilization since the wars with Ethiopia. According to a US State Department information sheet from October 2007 Eritrean national service consists of "approximately six months of military training, followed by a number of years in military or other government service." Besides national defense conscripts also spend peacetime working on public construction projects. They get paid no more than $30 a month. There is no term limit for national service, having been made open-ended in 1998.

USACE "USACE Information sheet on RIO" . There is also controversy about the Defense Contract Audit Agency (DCAA) report on billing methods for meals in which the auditors allegedly knew about, but disregarded, the Army requirement that directed KBR to have varying amounts of meals prepared at certain locations without respect to how many people used the service. Although KBR paid for the food, the DCAA did not believe they should be able to charge the DoD for meals prepared but not served. In June 2008, Charles M. Smith, the senior civilian Defense Department official overseeing the government's multibillion-dollar contract with KBR during the early stages of the war in Iraq said he was forced out of his job in 2004 for refusing to approve $1 billion in questionable charges by KBR.
In 1937, Bill Jr. was cast in an important featured role in a Monogram Tom Keene western, Romance of the Rockies, followed by a bit in the short subject, Our Gang Follies of 1938. A potentially major development was his signing to appear in MGM's Girl of the Golden West, portraying Nelson Eddy's role as a child in the opening sequences. The biographical information sheet filled out for young Cody upon his arrival at MGM in February, 1938, indicated that he was a junior high school student who enjoyed history, was an active Boy Scout, lived with his parents and brother in a California bungalow, enjoyed eating filet mignon, liked to swim and play baseball, had been interviewed on NBC, and had "made personal appearances with Daddy since 3 years old." The satellite giants-which combined serve about 30 million pay-TV subscribers-convinced FCC officials that they lacked the channel capacity to provide every eligible station in HD immediately.

The first gay publication was Burnt Offering, also published as Gay Forum, both in 1974, more of a manifesto-cum-pamphlet than a journal. Brian Gilmore produced a GLS Information Sheet on a weekly (term- time) basis for years. A formal ‘official’ publication was felt necessary, and Brian Gilmore became editor of NIGRA News, and then a member of the Collective that produced Northern Gay. Others in the Collective were: Jeff Dudgeon, a regular contributor to all of Northern Ireland’s gay magazines, John Lyttle, Stella Mahon of Sappho a short story writer, and employee of the Open University, Richard Kennedy then-President of NIGRA, and Michael Workman, who became a BBC journalist. These two journals were information sheets, but also carried in-depth articles, Northern Gay tended to have thematic editions on, for example, ‘coming out’, the law, and women's issues.

Wills & Sons acquired a fleet of thirteen industrial locomotives for the work, and also a number of wagons for the internal conveyance of materials from the gravel pits and from the jetty; materials arriving via the main line railway were conveyed in the carriers' wagons, but these could not be detained on the Estate network unless a demurrage charge was paid.
The Armstrong Whitworth was manufactured from 1904, when the company decided to diversify to compensate for a fall in demand for artillery after the end of the Boer War.Tyne & Wear County Museums, Undated, Information Sheet Armstrong Whitworth Car, available here and reverse here . A 1986 study by W.E. Kochanov of the Pennsylvania Geological Survey showed over 40 symbols representing surface depressions in the Green Pond Marsh area. A search of the Pennsylvania Department of Conservation and Natural Resources' online map viewer, when switched to "geology" view, indicated on February 8, 2015 that there were 13 sinkholes and 82 surface depressions within a one-half mile radius of the clicked marker. In a 2005 information sheet on sinkholes, the Pennsylvania Department of Environmental Resources states that "[s]inkhole occurrences will likely become more frequent as population encroaches upon karst areas traditionally used for farmland". While homeowners in karst locations are not explicitly advised by the Pennsylvania Department of Environmental Resources to purchase sinkhole insurance, the sheet recommends becoming aware of the risks of construction in karst geological areas and what assistance is available from the state, if any, to mitigate the risk.

On 11 March 1998, using a three-month observation arc, a faulty International Astronomical Union circular and press information sheet were put out that incorrectly concluded "that the asteroid was 'virtually certain' to pass within 80% of the distance to the Moon and stood a 'small...not entirely out of the question' possibility of hitting the Earth in 2028." But by 23 December 1997, it should have been clear that XF11 had no reasonable possibility of an Earth impact. Within hours of the announcement, independent calculations by Paul Chodas, Don Yeomans, and Karri Muinonen had calculated that the probability of Earth impact was essentially zero, and vastly less than the probability of impact from as-yet- undiscovered asteroids. Chodas (1999) concurs with Marsden (1999) that there was about 1 chance in a hundred thousand that XF11 could have passed through a keyhole—that is, until the 1990 precovery observations eliminated such possibilities.
Blue Scout Junior The XRM-91 Blue Scout Junior (sometimes called Journeyman B) was a rocket of the U.S. Air Force's System 609A Blue Scout family.Scout information sheet at astronautix.com (accessed 2008-10-22) Although the Blue Scout Junior had sufficient impulse to have put a small satellite in low Earth orbit, it was not used as an orbital launch vehicle. The XRM-91 did not resemble the other Scout variants externally, because the usual first Scout stage (an Aerojet General Algol) was not used. Instead, the four-stage Blue Scout Junior used Scout's 2nd and 3rd stages (Castor and Antares) as the first two stages, and added an Aerojet General Alcor and a spherical NOTS Cetus in a common nose fairing. The XRM-91 also lacked the gyro-stabilization and guidance system of the RM-89 Blue Scout I and RM-90 Blue Scout II, making it a completely unguided rocket.
Pensioners' Barracks in 1905 designed by Jewell in 1863 Jewell's duties initially included overseeing repairs of buildings, roads and bridges, and supervising the construction of the boys' schools in Perth and Fremantle. He was soon given the task of designing major buildings such as Perth Gaol and the court-house. Jewell was said to have worked hard and long into the night with his book-keeping duties. In 1853 he wrote to Governor Fitzgerald: > I have the honour to submit for the consideration and approval off His > Excellency the Governor that he will be pleased to grant an allowance for > candles used in writing the Public accounts done after office hours, as it > is impossible to get the same performed during the day without neglecting > other duties...Western Australian Museum information sheet: "The Old Perth > Gaol" With the arrival of 300 convicts and ticket of leave persons, Perth had the labour necessary to commence building many substantial projects.
INFORMATION SHEET Deptford and the dockyards The Emancipation of the Dispossessed Deptford's northern section nearest the old docks contains areas of council housing, with some concentrations of people experiencing the forms of deprivation typically associated with the poverty of Inner London. Northern Deptford near the Thames, along with neighbouring New Cross, has been touted as "the new Shoreditch" by some journalists and estate agents paying attention to a trendy arts and music scene that is popular with students and artists. Shoreditch is a former working class area of East London that has a number of contemporary art galleries and is home to a number of creative and media companies To the south where Deptford rolls into the suburban spread of Brockley, the previously multi-occupancy Victorian houses are being gentrified by young city workers and urban professionals. Deptford has a growing Vietnamese community reflected in the number of restaurants in the area.

By royal order King Friedrich II of Prussia presented the plot to his valet, Johann Gottfried Donner, on 7 November 1751. Donner had the palace constructed from 1751 to 1753 following plans by the court architect Friedrich Feldmann. Donner and his family lived on the ground floor of the palace and on the first upper storey (bel étage) Donner installed a large, elegant hall for public festivities as well as a private residence, which was rented to the administrator of the Prussian mint, Johann Philipp Graumann. The attic was used to store and air bulk grain, and at the back of the lot Donner laid out a garden with an underground cold storage room (Eiskeller) and a carriage house, using the remaining space to run a flourishing trade in timber.BIM Berliner Immobilienmanagement information sheet, "Palais am Festungsgraben" The building as headquarters of the Prussian Finance Ministry (1930) After Donner’s death in 1787 the Prussian Finance Ministry purchased the palace and provided lodgings on the top floor for its senior officers.

The grounds were enclosed by an high iron fence and the show ring by a picket fence, and "at intervals along the boundary, magnificent trees give a cool and picturesque appearance to the surrounds" (Brisbane newspaper article cited in Thuringowa Library Heritage Services Information Sheet Number 12). During the severe economic depression of the early 1930s a flood of public support resulted in a range of planned fundraisers and benefits which ensured that the Show continued throughout the Depression. The Depression also gave rise to other community activities staged at the Showground, including a soup kitchen to cater for the large number of itinerant workers or for those just down on their luck, and as a camp ground for the 9th and 49th Battalions' reunion held in conjunction with the 1933 Townsville Show week. An increase of grounds use led to improved event facilities with additional seating offering trackside viewing provided via a series of eight-tiered stands. Constructed by local manufacturer Curley Bell during the late 1920s and early 1930s and later modified to incorporate safety rails, the viewing stands became affectionately known as the "Curley Bell's".
