Multiple colonic biopsies are taken in order to make the diagnosis. Histological features of colonic biopsies indicating microscopic colitis are: greater than 20 intraepithelial lymphocytes per 100 epithelial cells and, additionally, 10-20 μm of a thickened subepithelial collagen band in collagenous colitis.

The presence of goblet cells, called intestinal metaplasia, is necessary to make a diagnosis of Barrett's esophagus. This frequently occurs in the presence of other metaplastic columnar cells, but only the presence of goblet cells is diagnostic. The metaplasia is grossly visible through a gastroscope, but biopsy specimens must be examined under a microscope to determine whether cells are gastric or colonic in nature. Colonic metaplasia is usually identified by finding goblet cells in the epithelium and is necessary for the true diagnosis.
Colonic polypectomy is the removal of colorectal polyps in order to prevent them from turning cancerous. Method of removing a polyp with a snare Gastrointestinal polyps can be removed endoscopically through colonoscopy or esophagogastroduodenoscopy, or surgically if the polyp is too large to be removed endoscopically. The method used to perform colonic polypectomies during colonoscopy depends on the size, shape and histological type of the polyp to be removed. Many people with diverticulosis have minimal to no symptoms, and do not require any specific treatment. Colonic stimulants should be avoided. Treatments, like some colon cleansers, that cause hard stools, constipation, and straining, are not recommended.

FAP defined by the development of hundreds or thousands of polyps in the colon. Gardner syndrome is set apart as a subtype because, in addition to colonic polyps, there are also extra-colonic growths (both malignant and benign). There are many terms used to describe "APC-associated polyposis condition" including FAP, attenuated FAP, Gardner syndrome, Turcot syndrome, and gastric adenocarcinoma and proximal polyposis of the stomach (GAPPS). An ameboma, also known as an amebic granuloma, is a rare complication of Entamoeba histolytica infection, where in response to the infecting amoeba there is formation of annular colonic granulation, which results in a large local lesion of the bowel.
If the stem cells at the base of the crypt express PMS2, generally all several thousand cells of the crypt will also express PMS2. This is indicated by the brown color seen by immunostaining of PMS2 in most of the enterocytes in the crypt in panel A of the image in this section. Similar expression of ERCC4 (XPF) and ERCC1 occurs in the thousands of enterocytes in each colonic crypt of the normal colonic epithelium. For ERCC4 (XPF) (C), there is absent expression in most of the cell nuclei of the crypts and in the colonic lumen in this area of tissue, but detectable expression at the neck of some crypts. When PMS2 is reduced in colonic crypts in a field defect, it is most often associated with reduced expression of DNA repair enzymes ERCC1 and ERCC4 (XPF) as well (see images in this section).
In differentiated human colonic epithelial cells, Jak3 redistributed to basolateral surfaces and interacted with adherens junction (AJ) protein β-catenin. Jak3 expression in these cells was essential for AJ localization of β-catenin and maintenance of epithelial barrier functions. Collectively, these results demonstrate the essential role of Jak3 in the colon where it facilitated mucosal differentiation by promoting the expression of differentiation markers and enhanced colonic barrier functions through AJ localization of β-catenin. Prebiotics may be beneficial to inflammatory bowel disease or Crohn's disease through production of SCFA as nourishment for colonic walls, and mitigation of ulcerative colitis symptoms. The sudden addition of substantial quantities of prebiotics to the diet may result in an increase in fermentation, leading to increased gas production, bloating or bowel movement. Production of SCFA and fermentation quality are reduced during long-term diets of low fiber intake. Until bacterial flora are gradually established to rehabilitate or restore intestinal bacteria, nutrient absorption may be impaired and colonic transit time temporarily increased with a rapid addition of higher prebiotic intake.
Bisacodyl works by stimulating enteric nerves to cause peristalsis, i.e., colonic contractions. It is also a contact laxative; it increases fluid and salt secretion. The action of bisacodyl on the small intestine is negligible; stimulant laxatives mainly promote evacuation of the colon.

Another indication for use is the conservative management of acute colonic pseudo-obstruction, or Ogilvie's syndrome, in which patients get massive colonic dilatation in the absence of a true mechanical obstruction. Hospitals sometimes administer a solution containing neostigmine intravenously to delay the effects of envenomation through snakebite. Some promising research results have also been reported for administering the drug nasally as a snakebite treatment.
In 1936, Kellogg filed a petition for his invention of improvements to an "irrigating apparatus particularly adaptable for colonic irrigating, but susceptible of use for other irrigation treatments." The improved irrigator included features such as measuring the amount of liquid entering and exiting the colon as well as indicating and regulating the positive pressure of the pumped liquid. At the Battle Creek Sanitarium, these colonic irrigators were often used to shoot gallons of water up a patient's rectum into their colon, sometimes followed by a half-pint yogurt enema to aid in further cleaning. It has been suggested that multiple people would get this treatment at one time.
Thus, any SIBO breath test that collects longer than 60 minutes may be measuring colonic activity. Diagnostic criteria of 20 ppm hydrogen and/or methane changes within 90 or 120 minutes will have higher positive rates of SIBO but this will reflect colonic activity not jejunal metabolism. A one-hour SIBO breath test avoids false positive results by collecting breath up to 60 minutes. Positive diagnosis for a glucose SIBO breath test – glucose is absorbed by the digestive system so studies have shown it to be harder to diagnose distal end bacterial overgrowth since the glucose typically doesn't reach the colon before being absorbed.
Children who suffer from fecal incontinence after the repair of an imperforate anus are usually those born with a bad prognosis type of defect and severe associated defects (defect of the sacrum, poor muscle complex). However, such children can still achieve a good quality of life when treated with the bowel management program. Children operated on for imperforate anus and who suffer from fecal incontinence can be divided into two groups that require individualized treatment plans: Children with constipation (colonic hypomotility): No special diet or medications are necessary for children with colonic hypomotility, a type of constipation. Their tendency towards constipation helps them to remain clean between enemas.
On the basis of histopathology and by expression of specific markers, the colonic tumors in the mice were virtually identical to those in humans. In humans, characteristic aberrant changes in molecular markers are detected both in field defects surrounding cancers (from which the cancers arise) and within cancers. In the colonic tissues of mice fed diet plus DOC similar changes in biomarkers occurred. The average half-life of mitochondria in rats, depending on cell type, is between 9 and 24 days, and in mice is about 2 days. In humans it is likely that the half life of mitochondria is also a matter of days to weeks. A stem cell at the base of a colonic crypt that was largely MT-COI-deficient may compete with the other 4 or 5 stem cells to take over the stem cell niche. If this occurs, then the colonic crypt would be deficient in MT-COI in all 1700 to 5,000 cells, as is indicated for some crypts in panels A, B and D of the image.
Adverse effects are typically limited to gastrointestinal reactions and include abdominal pain or cramps, diarrhea, nausea, and vomiting. Regular use of senna products can lead to a characteristic brown pigmentation of the internal colonic wall seen on colonoscopy. This abnormal pigmentation is known as melanosis coli.
SCAD is diagnosed via colonoscopy, often incidentally during examination for unrelated concerns. Colonoscopy shows erythema of the colonic mucosa, which may be characterized by friability and exudate. The descending and sigmoid colon are typically involved. Thus, SLC26A3 may be the major apical chloride/base exchanger and is essential for the absorption of chloride in the colon. In addition, SLC26A3 regulates colonic crypt proliferation. Deletion of SLC26A3 results in chloride-rich diarrhea and is associated with compensatory adaptive up-regulation of ion-absorbing transporters.
These watershed areas are most vulnerable to ischemia when blood flow decreases, as they have the fewest vascular collaterals. The rectum receives blood from both the inferior mesenteric artery and the internal iliac artery; the rectum is rarely involved by colonic ischemia due to this dual blood supply.
Colon cleansing, also known as colon therapy, or colon hydrotherapy, or a colonic, or colonic irrigation encompasses a number of alternative medical therapies claimed to remove unspecified toxins from the colon and intestinal tract by removing supposed accumulations of feces. Colon cleansing in this context should not be confused with an enema which introduces fluid into the colon, often under mainstream medical supervision, for a limited number of purposes including severe constipation and medical imaging. Some forms of colon hydrotherapy use tubes to inject water, sometimes mixed with herbs or with other liquids, into the colon via the rectum using special equipment. Oral cleaning regimes use dietary fiber, herbs, dietary supplements, or laxatives.
A hand-sewn bowel anastomosis, in this case of the sigmoid colon A surgical anastomosis is a surgical technique used to make a new connection between two body structures that carry fluid, such as blood vessels or bowel. For example, an arterial anastomosis is used in vascular bypass and a colonic anastomosis is used to restore colonic continuity after the resection of colon cancer. A surgical anastomosis can be created using suture sewn by hand, mechanical staplers and biological glues, depending on the circumstances. While an anastomosis may be end-to-end, equally it could be performed side-to-side or end-to-side depending on the circumstances of the required reconstruction or bypass.

Diverticulosis is the condition of having multiple pouches (diverticula) in the colon that are not inflamed. These are outpockets of the colonic mucosa and submucosa through weaknesses of muscle layers in the colon wall. They typically cause no symptoms. Diverticular disease occurs when diverticula become inflamed, known as diverticulitis, or bleed.
Since SIBO occurs in the proximal intestine, breath samples should be collected only within 1 hour after lactulose ingestion. This truly reflects proximal intestinal bacterial activity, not distal or colonic activity. The same argument is true if glucose is the substrate. Lactulose is a carbohydrate that is not absorbed by humans.

Abundant SLC26A2 mRNA levels have been identified in the small and large intestine of mice, rats and humans. In the human colon, SLC26A2 is present in the upper third of the crypts, where it is directed toward the apical membrane. The physiological role of SLC26A2 in the human colon remains to be determined, but it likely represent the sulfate/oxalate exchanger that has been characterized in colonic apical membrane vesicle preparations and possibly plays an important role in sulfate transport in this tissue. In fact, impaired sulfation has been suggested to occur during the course of malignant transformation of colonic epithelial cells, and studies have shown that the growth rate of cancer cells was markedly enhanced when the transcription of SLC26A2 was suppressed.
Colon cancer provides one example of the mechanisms by which diet, the top factor listed in the table, is an external factor in cancer. The Western diet of African Americans in the United States is associated with a yearly colon cancer rate of 65 per 100,000 individuals, while the high fiber/low fat diet of rural Native Africans in South Africa is associated with a yearly colon cancer rate of <5 per 100,000. Feeding the Western diet for two weeks to Native Africans increased their secondary bile acids, including carcinogenic deoxycholic acid, by 400%, and also changed the colonic microbiota. Evidence reviewed by Sun and Kato indicates that differences in human colonic microbiota play an important role in the progression of colon cancer.

An ileosigmoid knot is a form of volvulus in which ileum wraps around the base of the sigmoid and passes beneath itself forming a knot. The exact cause of this condition is not known. Patients usually present with clinical features of colonic obstruction. Vomiting, abdominal distension, abdominal pain, blood stained stools are frequent symptoms.
C. Septicum derived gas gangrene has shown strong correlations with increased levels of malignancy. Diverticulitis, specifically colonic diverticulitis, is a gastrointestinal disease characterized by inflammation of abnormal pouches—diverticula—which can develop in the wall of the large intestine. Symptoms typically include lower abdominal pain of sudden onset, but the onset may also occur over a few days. There may also be nausea; and diarrhea or constipation. Fever or blood in the stool suggests a complication.
Complete removal of a SSL is considered curative. Several SSLs confer a higher risk of subsequently finding colorectal cancer and warrant more frequent surveillance. Antibiotic-associated diarrhea (AAD) results from an imbalance in the colonic microbiota caused by antibiotics. Microbiotal alteration changes carbohydrate metabolism with decreased short-chain fatty acid absorption and an osmotic diarrhea as a result. Another consequence of antibiotic therapy leading to diarrhea is overgrowth of potentially pathogenic organisms such as Clostridium difficile. It is defined as frequent loose and watery stools with no other complications.
Several studies have measured the intestinal absorption of fructose using the hydrogen breath test. These studies indicate that fructose is not completely absorbed in the small intestine. When fructose is not absorbed in the small intestine, it is transported into the large intestine, where it is fermented by the colonic flora. Hydrogen is produced during the fermentation process and dissolves into the blood of the portal vein.
However, at least one review of the literature questions the existence of this vessel, with some experts calling for the abolition of these terms from future medical literature. Venous drainage usually mirrors colonic arterial supply, with the inferior mesenteric vein draining into the splenic vein, and the superior mesenteric vein joining the splenic vein to form the hepatic portal vein that then enters the liver.

Once ingested, aloin increases peristaltic contractions in the colon, which induces bowel movements. Aloin also prevents the colon from re-absorbing water from the gastrointestinal tract, which leads to softer stools. This effect is caused by aloin's opening of chloride channels of the colonic membrane. In higher doses, these effects may lead to electrolyte imbalance, diarrhea, and abdominal pain, which are common side-effects of the drug.
A positive test is almost always an indication to do a colonoscopy. In most cases the positive result is just due to hemorrhoids; however, it can also be due to diverticulosis, inflammatory bowel disease (Crohn's disease, ulcerative colitis), colon cancer, or polyps. This artery is found low in the mesentery, near the root. In the setting of chronic ischemic colitis, both the marginal artery and the meandering mesenteric artery may be enlarged significantly, and may provide significant blood flow to the ischemic colonic segment.
This is called esophagitis, gastritis, duodenitis, ileitis, and colitis depending on the parts affected. It can be due to infections or other conditions, including coeliac disease, and inflammatory bowel disease affects the layers of the gastrointestinal tract in different ways. Ulcerative colitis involves the colonic mucosa. Crohn's disease may produce inflammation in all layers in any part of the gastrointestinal tract and so can result in transmural fistulae.
Moderate to severe disease correlates with more than six stools daily, frequent bloody stools and urgency. Moderate abdominal pain, low-grade fever, , and anemia may develop (not requiring transfusion). Toxicity is present, as demonstrated by fever, tachycardia, anemia or an elevated ESR or CRP. Fulminant disease correlates with more than 10 bowel movements daily, continuous bleeding, toxicity, abdominal tenderness and distension, blood transfusion requirement, and colonic dilation (expansion).
In healthy individuals, homeostasis is maintained when cellular uptake and kidney excretion naturally counterbalance a patient's dietary intake of potassium. When kidney function becomes compromised, the ability of the body to effectively regulate serum potassium via the kidney declines. To compensate for this deficit in function, the colon increases its potassium secretion as part of an adaptive response. However, serum potassium remains elevated as the colonic compensating mechanism reaches its limits.

The pigments are absorbed in the colon. Betalains are oxidation-sensitive redox indicators that are decolorized by hydrochloric acid, ferric ions, and colonic bacteria preparations. The gut flora play a not-yet-evaluated role in the breakdown of the pigment.
Allbritton's interest in single-cell analysis have hinged on the use of capillary electrophoresis (CE) and microfabricated technologies. Through this work she has studied lipid signaling at the single-cell level, the isolation cytotoxic t-cells with specific properties, and the capture of colonic crypts. ERβ is expressed by many tissues including the uterus, blood monocytes and tissue macrophages, colonic and pulmonary epithelial cells and in prostatic epithelium and in malignant counterparts of these tissues. Also, ERβ is found throughout the brain at different concentrations in different neuron clusters. ERβ is also highly expressed in normal breast epithelium, although its expression declines with cancer progression. ERβ is expressed in all subtypes of breast cancer.

Secondary bile acids increase intracellular production of reactive oxygen and reactive nitrogen species resulting in increased oxidative stress and DNA damage. As shown in the figure in this section, deoxycholate added to the diet of mice increased the level of 8-oxo-dG, an oxidative DNA damage, in the colonic epithelium of mice.
Another suggestion was that cells with a deficiency in cytochrome c oxidase are apoptosis resistant, and thus more likely to survive. The linkage of MT-COI to apoptosis arises because active cytochrome c oxidase oxidizes cytochrome c, which then activates pro-caspase 9, leading to apoptosis. These two factors may contribute to the frequent occurrence of MT-COI-deficient colonic crypts with age or during carcinogenesis in the human colon.

The blood supply from these two major arteries overlap, with abundant collateral circulation via the marginal artery of the colon. Diverticulosis often occurs in the sigmoid colon in association with increased intraluminal pressure and focal weakness in the colonic wall. It is a common cause of hematochezia. Volvulus occurs when a portion of the bowel twists around its mesentery, which can lead to obstruction and infarction. Volvulus in the elderly commonly occurs in the sigmoid colon, whereas in infants and children it is more likely to occur in the midgut.

The gastrocolic reflex can also be used to optimise the treatment of constipation. Since the reflex is most active in the mornings and immediately after meals, consumption of stimulant laxatives, such as sennosides and bisacodyl, during these times will augment the reflex and help increase colonic contractions and therefore defecation.
Antibiotics are a common treatment for children, with 11% to 40% of antibiotic-treated children developing diarrhea. Antibiotic-associated diarrhea (AAD) results from an imbalance in the colonic microbiota caused by antibiotic therapy. These microbial community alterations result in changes in carbohydrate metabolism, with decreased short-chain fatty acid absorption and osmotic diarrhea as a result. A 2015 Cochrane review concluded that a protective effect of some probiotics existed for AAD in children.
The two inflammatory conditions are quite indistinguishable based on physical manifestations. Patients with diverticulitis will present with nausea, vomiting, fever, elevated leukocyte count rebound tenderness, and will have more extensive lower abdominal pain than patients with epiploic appendagitis. Additionally inflammation from diverticulitis may spread to the epiploic appendages making it difficult to diagnose, for inflammation of the appendices epiploicae may be resultant to other inflammatory conditions in the colonic wall and surrounding mesocolon.
Research suggests soy-derived lecithin has significant effects on lowering serum cholesterol and triglycerides, while increasing HDL ("good cholesterol") levels in the blood of rats. However, a growing body of evidence indicates lecithin is converted by gut bacteria into trimethylamine N-oxide (TMAO), which is absorbed by the gut and may with time contribute to atherosclerosis and heart attacks.Wendy R Russell WR et al. Bacteria that are members of the 'normal flora' of the region of the infection are often also isolated from lesions involving anaerobic bacteria. Pilomatricomas have been observed in a variety of genetic disorders including Turner syndrome, myotonic dystrophy, Rubinstein-Taybi syndrome, Trisomy 9, and Gardner syndrome. It has been reported that the prevalence of pilomatricomas in Turner syndrome is 2.6%. Hybrid cysts that are composed of epidermal inclusion cysts and pilomatricoma-like changes have been repeatedly observed in Gardner syndrome. This association has prognostic import, since cutaneous findings in children with Gardner Syndrome generally precede colonic polyposis.
Several hypotheses for the association were proposed, the most prominent being the idea that there is no causal relationship between aortic stenosis and gastrointestinal bleeding, they are both just common conditions in the elderly, and they sometimes overlap. Other hypotheses included hypoxia of the colonic mucosa and bowel ischemia due to low blood flow, both of which were discounted by later research. Another early hypothesis of note was proposed by Greenstein et al. in 1986.
There are two main types of cystitis glandularis, non-mucinous and mucinous (intestinal). The difference is in the cellular production of mucin, a normal feature of colonic and intestinal epithelial cells but not of urothelial cells. Another distinction is made between focal areas and diffuse involvement of the bladder. The increased biodiversity and different composition of gut flora in African populations may aid in the digestion of normally indigestible plant polysaccharides and also may result in a reduced incidence of non- infectious colonic diseases. On a smaller scale, it has been shown that sharing numerous common environmental exposures in a family is a strong determinant of individual microbiome composition. This effect has no genetic influence and it is consistently observed in culturally different populations.

Probiotics (either as pills or as yogurt) do not appear to decrease the rate of occurrence of vaginal yeast infections. No benefit has been found for active infections. Example probiotics purported to treat and prevent candida infections are Lactobacillus fermentum RC-14, Lactobacillus fermentum B-54, Lactobacillus rhamnosus GR-1, Lactobacillus rhamnosus GG and Lactobacillus acidophilus. There is no evidence to support the use of special cleansing diets and colonic hydrotherapy for prevention.
Based on these measurements, crypts have between 1725 and 2530 cells. Another report gave a range of 1500 to 4900 cells per colonic crypt. The occurrence of frequent crypts with almost complete loss of MT-COI in their 1700 to 5,000 cells suggests a process of natural selection. About 20% of patients with acute ischemic colitis may develop a long-term complication known as chronic ischemic colitis. Symptoms can include recurrent infections, bloody diarrhea, weight loss, and chronic abdominal pain. Chronic ischemic colitis is often treated with surgical removal of the chronically diseased portion of the bowel. A colonic stricture is a band of scar tissue which forms as a result of the ischemic injury and narrows the lumen of the colon.
Some examples exist of factors that can cause the lactase persistence phenotype in the absence of any genetic variant associated with LP. Individuals may lack the alleles for lactase persistence, but still tolerate dairy products in which lactose is broken down by the fermentation process (e.g. cheese, yogurt). Also, healthy colonic gut bacteria may also aid in the breakdown of lactose, allowing those without the genetics for lactase persistence to gain the benefits from milk consumption.
Ogilvie syndrome is the acute dilatation of the colon in the absence of any mechanical obstruction in severely ill patients. Acute colonic pseudo- obstruction is characterized by massive dilatation of the cecum (diameter > 10 cm) and right colon on abdominal X-ray. It is a type of megacolon, sometimes referred to as "acute megacolon," to distinguish it from toxic megacolon. The condition carries the name of the British surgeon Sir (1887–1971), who first reported it in 1948.

The enteric nervous system is independent but is connected to the central nervous system by sympathetic and parasympathetic nerves. The targets of the enteric neurons are muscle cells, secretory cells, endocrine cells, microvasculature, and inflammatory cells. Observational studies show a strong association between delayed intestinal transit and the production of methane. Experimental data suggest a direct inhibitory activity of methane on the colonic and ileal smooth muscle and a possible role for methane as a gasotransmitter. Statins can inhibit archaeal cell membrane biosynthesis apparently without affecting bacterial numbers as demonstrated in livestock and humans. This opens the possibility of a therapeutic intervention that targets a specific etiological factor of constipation while protecting the intestinal microbiome.

Diverticulitis is a common cause of abdominal pain resulting from outpouchings that particularly affects the colon. Functional colonic diseases refer to disorders without a known cause, and include irritable bowel syndrome and intestinal pseudoobstruction. Constipation may result from lifestyle factors, impaction of a rigid stool in the rectum, or in neonates, Hirschprung's disease. Diseases affecting the large intestine may cause blood to be passed with stool, may cause constipation, or may result in abdominal pain or a fever.
Regular consumption of dairy foods containing lactose can promote a colonic bacteria adaptation, enhancing a favorable microbiome, which allows people with primary lactase deficiency to diminish their intolerance and to consume more dairy foods. Short-chain fatty acids, which include butyric acid, are produced by beneficial colonic bacteria (probiotics) that feed on, or ferment prebiotics, which are plant products that contain dietary fiber. These short-chain fatty acids benefit the colonocytes by increasing energy production, and may protect against colon cancer by inhibiting cell proliferation. Conversely, some researchers have sought to eliminate butyrate and consider it a potential cancer driver. Studies in mice indicate it drives transformation of MSH2-deficient colon epithelial cells.
As stated in the previous section, butyrate is involved with multiple cellular functions. Because of its involvement with these functions, it is hypothesized that butyrate can act as a protective agent against colon cancer and various inflammatory bowel diseases. Butyrate plays a key role in colon cancer by switching its role concerning cellular proliferation and apoptosis depending on the state and conditions of the cell. Butyrate also possesses anti-inflammatory effects to decrease colonic inflammation such as ulcerative colitis.

The absence or depletion of these BPB in the gut could therefore be a possible aide in the overly-active inflammatory response. Butyrate in the gut also protects the integrity of the intestinal epithelial barrier. Decreased butyrate levels therefore lead to a damaged or dysfunctional intestinal epithelial barrier. In a 2013 research study conducted by Furusawa et al, microbe-derived butyrate was found to be essential in inducing the differentiation of colonic regulatory T cells in mice.

Copper deficiency myelopathy in humans was discovered and first described by Schleper and Stuerenburg in 2001. They described a patient with a history of gastrectomy and partial colonic resection who presented with severe tetraparesis and painful paraesthesias and who was found on imaging to have dorsomedial cervical cord T2 hyperintensity. Upon further analysis, it was found that the patient had decreased levels of serum coeruloplasmin, serum copper, and CSF copper. The patient was treated with parenteral copper and the patient's paraesthesias did resolve.
Toxic megacolon is an acute form of colonic distension. It is characterized by a very dilated colon (megacolon), accompanied by abdominal distension (bloating), and sometimes fever, abdominal pain, or shock. Toxic megacolon is usually a complication of inflammatory bowel disease, such as ulcerative colitis and, more rarely, Crohn's disease, and of some infections of the colon, including Clostridium difficile infections, which have led to pseudomembranous colitis. Other forms of megacolon exist and can be congenital (present since birth, such as Hirschsprung's disease).
The most common type of skin manifestation, erythema nodosum, presents as raised, tender red nodules usually appearing on the shins (extensor surfaces). Erythema nodosum is due to inflammation of the underlying subcutaneous tissue (panniculitis). A more severe skin manifestation, pyoderma gangrenosum, is characterized painful pustules or nodules that become ulcers which progressively grow. Whereas erythema nodosum tends correlate with the activity of the ulcerative colitis and often improves with treatment of the colonic inflammation, pyoderma gangrenosum may occur independently of UC disease activity.
In some cases, primary sclerosing cholangitis occurs several years before the bowel symptoms of ulcerative colitis develop. PSC does not parallel the onset, extent, duration, or activity of the colonic inflammation in ulcerative colitis. In addition, colectomy does not have an impact on the course of primary sclerosing cholangitis in individuals with UC. PSC is associated with an increased risk of colorectal cancer and cholangiocarcinoma (bile duct cancer). PSC is a progressive condition, and may result in cirrhosis of the liver.
It is prevalent in Bangladesh, India, Burma, China, Kazakhstan, Philippines, Thailand, Vietnam, and the Volga Delta of Russia, with isolated cases from Africa, such as Nigeria. It is especially notable in the Assam, Bengal, Bihar, Madhya Pradesh, Orissa and Uttar Pradesh regions of India. It is also known as the colonic fluke, particularly when infecting other animals. Its natural habitat is the colon of pigs, and has also been found in rhesus monkeys, orang-utans, fish, field rats and Napu mouse-deer.
A high-fiber diet and fiber supplements are advisable to prevent constipation. The American Dietetic Association recommends 20–35 grams each day. Wheat bran has been shown to reduce intra colonic pressure. The US National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) says foods such as nuts, popcorn hulls, sunflower seeds, pumpkin seeds, caraway seeds, and sesame seeds have traditionally been labeled as problem foods for people with this condition; however, no scientific data exists to prove this hypothesis.
Potassium can bind to a number of agents in the gastrointestinal tract. Sodium polystyrene sulfonate with sorbitol (Kayexalate) has been approved for this use and can be given by mouth or rectally. However, high quality evidence to demonstrate the effectiveness of sodium polystyrene are lacking, and use of sodium polystyrene sulfonate, particularly with high sorbitol content, is uncommonly but convincingly associated with colonic necrosis. There are no systematic studies (>6 months) looking at the long-term safety of this medication.
Metabolic activation is required for PhIP to function as a mutagen. Therefore, the cancer risk posed by PhIP depends on the extent at which PhIP is metabolized. After absorption, PhIP is converted to a genotoxic metabolite in the liver by Phase I enzyme N-oxidation by Cytochrome P-450 1A2 (CYP1A2). PhIP can be further metabolized into a more potent metabolite through O-acetylation by hepatic or colonic N-acetyltransferase 1 (NAT1) and N-acetyltransferase 2 (NAT2), or by sulfotransfereases.
Because colonic bacteria synthesize a significant portion of the Vitamin K required for human needs, individuals with disruptions to or insufficient amounts of these bacteria can be at risk for Vitamin K deficiency. Newborns, as mentioned above, fit into this category, as their colons are frequently not adequately colonized in the first five to seven days of life. Another at-risk population comprises those individuals on any sort of long-term antibiotic therapy, as this can diminish the population of normal gut flora.

Elderly: diverticulitis, intestinal obstruction, colonic carcinoma, mesenteric ischemia, leaking aortic aneurysm. The term "" is used to describe a condition mimicking appendicitis.
One member of the SulP family, SLC26A3, has been knocked out in mice. Apical membrane chloride/base exchange activity was sharply reduced, and the luminal content was more acidic in SLC26A3-null mouse colon. The epithelial cells in the colon displayed unique adaptive regulation of ion transporters; NHE3 expression was enhanced in the proximal and distal colon, whereas colonic H+/K+-ATPase and the epithelial sodium channel showed massive up-regulation in the distal colon. Plasma aldosterone was increased in SLC26A3-null mice.
Colic in horses is defined as abdominal pain, but it is a clinical symptom rather than a diagnosis. The term colic can encompass all forms of gastrointestinal conditions which cause pain as well as other causes of abdominal pain not involving the gastrointestinal tract. The most common forms of colic are gastrointestinal in nature and are most often related to colonic disturbance. There are a variety of different causes of colic, some of which can prove fatal without surgical intervention.
Histological section. The large intestine absorbs water and any remaining absorbable nutrients from the food before sending the indigestible matter to the rectum. The colon absorbs vitamins that are created by the colonic bacteria, such as vitamin K (especially important as the daily ingestion of vitamin K is not normally enough to maintain adequate blood coagulation), thiamine and riboflavin. It also compacts feces, and stores fecal matter in the rectum until it can be discharged via the anus in defecation.
Fecal microbiota transplant, also known as a stool transplant, is roughly 85% to 90% effective in those for whom antibiotics have not worked. It involves infusion of the microbiota acquired from the feces of a healthy donor to reverse the bacterial imbalance responsible for the recurring nature of the infection. The procedure replenishes the normal colonic microbiota that had been wiped out by antibiotics, and re-establishes resistance to colonization by Clostridioides difficile. Side effects, at least initially, are few.
Resistant starch does not release glucose within the small intestine, but rather reaches the large intestine where it is consumed or fermented by colonic bacteria (gut microbiota). On a daily basis, human intestinal microbiota encounter more carbohydrates than any other dietary component. This includes resistant starch, non-starch polysaccharide fibers, oligosaccharides, and simple sugars which have significance in colon health. The fermentation of resistant starch produces short-chain fatty acids, including acetate, propionate, and butyrate and increased bacterial cell mass.
A bowel polyp that can be identified by sigmoidoscopy. Some polyps will develop into cancers if not removed. Screening for colorectal cancer, if done early enough, is preventive because almost all colorectal cancers originate from benign growths called polyps, which can be located and removed during a colonoscopy (see colonic polypectomy). The US Preventive Services Task Force recommends screening for colorectal cancer using fecal occult blood testing, sigmoidoscopy, or colonoscopy, in adults, beginning at age 50 years and continuing until age 75 years.
There may be an important association between foetal distress and hypoxia with MSAF. It is believed that foetal distress develops into foetal hypoxia causing the foetus to defecate meconium resulting in MSAF and then perhaps MAS. Other stressors which causes foetal distress, and therefore meconium passage, includes when umbilical vein oxygen saturation is below 30%. Foetal hypoxic stress during parturition can stimulate colonic activity, by enhancing intestinal peristalsis and relaxing the anal sphincter, which results in the passage of meconium.
The second purpose is to secrete mucus into the colon lumen to lubricate the now dehydrated feces. This is crucial as a failure to lubricate the feces can result in colonic damage by the feces as it passes towards the rectum. When these glands undergo a number of changes at the genetic level, they proceed in a predictable manner as they move from benign to an invasive, malignant colon cancer. Colonic crypts deficient for CCOI reaches, on average, 18% in women and 23% in men, by 80–84 years of age. Crypts of the colon can reproduce by fission, as seen in panel C, where a crypt is dividing to form two crypts, and in panel B where at least one crypt appears to be fissioning. Most crypts deficient in CCOI are in clusters of crypts (clones of crypts) with two or more CCOI-deficient crypts adjacent to each other (see panel D).
These studies found that the region was linked to increased risks for a variety of cancers, notably in the prostate, breast, ovaries, colonic, and pancreas. Using inserts of the gene desert into bacterial artificial chromosomes, one study was able to produce enhancer activity in certain regions, which were isolated via cloning systems. In pigs T. suis egg production is sporadic, complicating diagnosis by fecal flotation. Necropsy of clinical cases of trichuriasis may be necessary to validate a diagnosis, since clinical signs may develop prior to patency, thus inhibiting diagnosis by fecal examination alone. On gross necropsy, the intestine may be filled with semisolid to watery to bloody mucoid feces, depending on acuteness of the infection and simultaneous bacterial infections. The anterior portion of adult worms may be observable breaching the cecal and colonic mucosa.
Duodenography and colonography are performed like a standard abdominal examination using B-mode and color flow Doppler ultrasonography using a low frequency transducer — for example a 2.5MHz — and a high frequency transducer, for example a 7.5MHz probe. Detailed examination of duodenal walls and folds, colonic walls and haustra was performed using a 7.5MHz probe. Deeply located abdominal structures were examined using 2.5MHz probe. All ultrasound examinations are performed after overnight fasting (for at least 16 hours) using standard scanning procedure.
Thus, a decrease in expression of MBD4 could cause an increase in carcinogenic mutations. MBD4 expression is reduced in almost all colorectal neoplasms due to methylation of the promoter region of MBD4. Also MBD4 is deficient due to mutation in about 4% of colorectal cancers. A majority of histologically normal fields surrounding neoplastic growths (adenomas and colon cancers) in the colon also show reduced MBD4 mRNA expression (a field defect) compared to histologically normal tissue from individuals who never had a colonic neoplasm.
Thus, exposure of the colon to deoxycholate may cause cancer in mice. In humans, higher levels of colonic deoxycholate are associated with higher frequencies of colon cancer. As an example, the fecal deoxycholate concentrations in African Americans (who eat a relatively high fat diet) is more than five times higher than fecal deoxycholate of Native Africans in South Africa (who eat a low fat diet). Male African Americans have a high incidence of colon cancer of 72 per 100,000,American Cancer Society.
The gastroileal reflex is one of the three extrinsic reflexes of the gastrointestinal tract, the other two being the gastrocolic reflex and the enterogastric reflex. The gastroileal reflex is stimulated by the presence of food in the stomach and gastric peristalsis. Initiation of the reflex causes peristalsis in the ileum and the opening of the ileocecal valve (which allows the emptying of the ileal contents into the large intestine, or colon). This in turn stimulates colonic peristalsis and an urge to defecate.
It has been shown that butyrate inhibits activity of HDAC1 that is bound to the Fas gene promoter in T cells, resulting in hyperacetylation of the Fas promoter and upregulation of Fas receptor on the T cell surface. It is thus suggested that butyrate enhances apoptosis of T cells in the colonic tissue and thereby eliminates the source of inflammation (IFN-γ production). Butyrate inhibits angiogenesis by inactivating Sp1 transcription factor activity and downregulating vascular endothelial growth factor gene expression.
Mannitol and large volume of saline were first used as bowel preparation agents prior to colonoscopy. As the use of Mannitol causes the production of methane, hydrogen, and other flammable gases, it was reported to be associated with colonic explosion. Large volume of saline was also reported to significantly impact the electrolyte balance and net fluid within the body. Later in the 1990, the polyethylene-glycol electrolyte lavage solution or PEG- ELS, was formulated with more effectiveness and safety to use.
The test is very accurate. In a series of 49 patients who underwent surgery for colovesical fistula due to sigmoid diverticulitis, the poppy seed test gave a correct diagnosis more often than abdominopelvic computerized tomography, magnetic resonance tomography of the abdomen, cystogram, retrograde colonic enema, urethrocystoscopy, and colonoscopy. In a series of 20 patients in the United States, the poppy seed test was significantly more accurate than computed tomography. In these two series, respectively, sensitivity of the test was 94.6% and 100%.
The genus Cinnamomum comprises about 300 species, of which four species are used to obtain the spice 'cinnamon'. 'Ceylon/True' cinnamon (Cinnamomum zeylancium) and Chinese Cassia cinnamon (Cinnamomum aromaticum) are the most widely available varieties. Studies have demonstrated many beneficial health effects of cinnamon, such as anti- inflammatory properties, anti-microbial activity, blood glucose control, reducing cardiovascular disease, boosting cognitive function and reducing risk of colonic cancer. Also, there is a study about cinnamon can be a pharmaceutical agent in diabetes.
Another study showed that bismuth acted synergistically with various antibiotics to inhibit sulfate-reducing gut bacteria and sulfide production. Some authors proposed a theory that hydrogen sulfide was involved in the development of ulcerative colitis and that bismuth might be helpful in the management of this condition. However, bismuth administration in rats did not prevent them from developing ulcerative colitis despite reduced hydrogen sulfide production. Also, evidence suggests that colonic hydrogen sulfide is largely present in bound forms, probably sulfides of iron and other metals.

Under certain circumstances, when more concentrated, as in the gallbladder, cholesterol crystallises and is the major constituent of most gallstones (lecithin and bilirubin gallstones also occur, but less frequently). Every day, up to 1 g of cholesterol enters the colon. This cholesterol originates from the diet, bile, and desquamated intestinal cells, and can be metabolized by the colonic bacteria.
Krusen realized that the growth of the field of physical medicine required an ongoing commitment to research, and he was an early proponent of outcome studies and evidence-based practice. His scholarly output was abundant, with publication of more than 400 scientific articles and many textbooks throughout his career. Krusen is best known for his work on physical modalities, with more than 100 articles in that topical area. In the 1930s, the modality articles focused on fever therapy, shortwave diathermy, heat lamps, colonic irrigation, and radiotherapy.

Gordon's early career focused on the development of cell lineages within the gastrointestinal tract. His laboratory initially combined the use of transgenic mouse models and biochemical approaches to elucidate the mechanisms of gut epithelial development along the duodenal-colonic and crypt- villus axes. Early studies also provided important insight into biochemical properties of lipid handling and transport in the digestive system. Dr. Gordon and colleagues later combined laser capture microdissection, and functional genomics to characterize specified cell populations within the gastrointestinal tract, including multipotent stem cells.
When ERCC4 (XPF) is reduced in colonic crypts in a field defect, it is most often associated with reduced expression of DNA repair enzymes ERCC1 and PMS2 as well, as illustrated in the image in this section. Deficiencies in ERCC1 (XPF) in colon epithelium appear to be due to epigenetic repression. A deficiency of ERCC4 (XPF) would lead to reduced repair of DNA damages. As indicated by Harper and Elledge, defects in the ability to properly respond to and repair DNA damage underlie many forms of cancer.
It was developed as the potential treatment of several conditions including familial adenomatous polyposis (FAP), precancerous sporadic colonic polyps, cervical dysplasia and the prevention of tumor recurrence in prostate and breast cancer. Exisulind inhibits the enzyme cGMP-PDE, overexpressed in precancerous and cancerous colorectal cells, and induces apoptosis in such cells with minimal effects on normal cells. This apoptotic effect is independent of COX-1 or COX-2 inhibition, p53, Bcl-2, or cell cycle arrest. Preclinical evidence suggests that exisulind also inhibits angiogenesis.
Indeed, recent work has shown that MORT is epigenetically silenced in both DCIS, a premalignant lesion of invasive breast cancer, and colonic adenomas, a premalignant lesion of colon adenocarcinoma. Furthermore epigenetic silencing of MORT is associated with luminal, hormone receptor positive breast cancer, overexpression of the oncogene CCND1, and GATA3 mutations, but is negatively correlated with p53 mutations. In summary, aberrant DNA hypermethylation- mediated epigenetic silencing of MORT occurs early during human carcinogenesis apparently coincident with when a mortal cell pathologically transitions to an immortal cell.
It has a secondary laxative effect in the colon, where it is fermented by the gut flora, producing metabolites which have osmotic powers and peristalsis- stimulating effects (such as acetate), but also methane associated with flatulence. Lactulose is metabolized in the colon by bacterial flora to short- chain fatty acids, including lactic acid and acetic acid. These partially dissociate, acidifying the colonic contents (increasing the H+ concentration in the gut). This favors the formation of the nonabsorbable from NH3, trapping NH3 in the colon and effectively reducing plasma NH3 concentrations.
In the liver, bilirubin is conjugated with glucuronic acid by the enzyme glucuronyltransferase, making it soluble in water: the conjugated version is the main form of bilirubin present in the "direct" bilirubin fraction. Much of it goes into the bile and thus out into the small intestine. Though most bile acid is reabsorbed in the terminal ileum to participate in enterohepatic circulation, conjugated bilirubin is not absorbed and instead passes into the colon. Full colonoscopy to the cecum and entry into the terminal ileum is attempted only if the diagnosis of UC is unclear. Otherwise, a flexible sigmoidoscopy is sufficient to support the diagnosis. The physician may elect to limit the extent of the exam if severe colitis is encountered to minimize the risk of perforation of the colon.
Firstly, being small molecules and either poorly absorbed or not absorbed at all, they drag water into the intestine via osmosis. Secondly, these molecules are readily fermented by colonic bacteria, so upon malabsorption in the small intestine they enter the large intestine where they generate gases (hydrogen, carbon dioxide and methane). The dual actions of these carbohydrates cause an expansion in volume of intestinal contents, which stretches the intestinal wall and stimulates nerves in the gut. Hypaque or other water- soluble dye may be placed through the passage to ensure patency of the stent on fluoroscopy. Enteric and colonic SEMS are inserted in a similar fashion, but in the duodenum and colon respectively. Biliary SEMS are used to palliatively treat tumours of the pancreas or bile duct that obstruct the common bile duct.
In cancers, loss of expression of genes occurs about 10 times more frequently by hypermethylation of promoter CpG islands than by mutations. For instance, in colon tumors compared to adjacent normal-appearing colonic mucosa, about 600 to 800 heavily methylated CpG islands occur in promoters of genes in the tumors while these CpG islands are not methylated in the adjacent mucosa. In contrast, as Vogelstein et al. point out, in a colorectal cancer there are typically only about 3 to 6 driver mutations and 33 to 66 hitchhiker or passenger mutations.
Segmental colitis associated with diverticulosis (SCAD) is a condition characterized by localized inflammation in the colon, which spares the rectum and is associated with multiple sac-like protrusions or pouches in the wall of the colon (diverticulosis). Unlike diverticulitis, SCAD involves inflammation of the colon between diverticula (interdiverticular mucosa), while sparing the diverticular orifices. SCAD may lead to abdominal pain, especially in the left lower quadrant, intermittent rectal bleeding and chronic diarrhea. The cause of SCAD is unknown, but may be related to local colonic ischemia, fecal stasis, or mucosal prolapse.
Another underrecognized and potentially life-threatening side effect spectrum is gastrointestinal hypomotility, which may manifest as severe constipation, fecal impaction, paralytic ileus, bowel obstruction, acute megacolon, ischemia or necrosis. Colonic hypomotility has been shown to occur in up to 80% of people prescribed clozapine when gastrointestinal function is measured objectively using radiopaque markers. Clozapine-induced gastrointestinal hypomotility currently has a higher mortality rate than the better known side effect of agranulocytosis. A Cochrane review found little evidence to help guide decisions about the best treatment for gastrointestinal hypomotility caused by clozapine and other antipsychotic medication.

Attenuated FAP can occur from other mutations in the APC gene, and causes a phenotype wherein colonic polyps may be few in number.

Treatments for colonic duplication varies from conservative management for asymptomatic cases to excision of duplicated colon to avoid potential issues such as colon structure and obstruction. Resection is possible when each duplicated colon has a complete blood supply. If the duplicated colons share a wall, a septotomy can be performed to create a small hole to connect two colons. In cases where rectum is also duplicated, either the rectums should be converted into one reservoir through septotomy followed by anorectoplasty or the duplicated colon and rectum should be removed and colostomies should be constructed.

Diverticular disease and adenomatous polyps was historically unknown and colon cancer was exceedingly rare in communities where appendicitis itself was rare or absent, such as various African communities. Studies have implicated a transition to a Western diet lower in fibre in rising frequencies of appendicitis as well as the other aforementioned colonic diseases in these communities. And acute appendicitis has been shown to occur antecedent to cancer in the colon and rectum. Several studies offer evidence that a low fiber intake is involved in the pathogenesis of appendicitis.
The diet of the prehistoric hunter-forager in the Chihuahuan Desert has been estimated to include 135 g per day of inulin-type fructans. Many foods naturally high in inulin or fructo-oligosaccharides, such as chicory, garlic, and leek, have been seen as "stimulants of good health" for centuries. Due to its resistance to digestive enzymes, inulin resists absorption during its transit through the upper gastrointestinal tract. After reaching the large intestine, inulin is converted by colonic bacteria to a gel known as a prebiotic, a food ingredient that is highly nourishing to gut microflora.
A 2014 meta-analysis of three small trials evaluating probiotics showed a slight improvement in management of chronic idiopathic constipation, but well-designed studies are necessary to know the true efficacy of probiotics in treating this condition. Children with functional constipation often claim to lack the sensation of the urge to defecate, and may be conditioned to avoid doing so due to a previous painful experience. One retrospective study showed that these children did indeed have the urge to defecate using colonic manometry, and suggested behavioral modification as a treatment for functional constipation.

TcdA can induce the physiological changes that occur in C. difficile related pseudomembranous colitis (PMC), a severe ulceration of the colon. Toxin damage to the colonic mucosa promotes accumulations of fibrin, mucin, and dead cells to form a layer of debris in the colon (pseudomembrane), causing an inflammatory response. TcdA damage causes increased epithelial permeability, cytokine and chemokine production, neutrophil infiltration, production of reactive oxygen species (ROS), mast cell activation, and direct damage to the intestinal mucosa. All can be attributed to TcdA induced inactivation of Rho GTPase proteins.
Todd Huntley Baron is an American gastroenterologist who teaches at the University of North Carolina School of Medicine. He is well known for his prolific publishing in the field of gastroenterology particularly in endoscopic retrograde cholangiopancreatography (ERCP), endoscopic ultrasound (EUS) and other advanced endoscopic procedures. His career has been based upon developing novel interventional endoscopic techniques for the care of patients with gastrointestinal, liver and other medical conditions. He was the first to describe endoscopic drainage of the gallbladder, placement of a colonic stent and endoscopic pancreatic necrosectomy.
Finally, the chromatin filaments emerging from these processes form a mass from which dozens of dome nuclei are amitotically generated (Fleming, 2015c)over a period of approximately 3 hours with the apparent involvement of nuclear envelope- limited sheets. Highly-fermentable fiber residues, such as those from resistant starch, oat bran, pectin, and guar are transformed by colonic bacteria into short-chain fatty acids (SCFA) including butyrate, producing more SCFA than less fermentable fibers such as celluloses. One study found that resistant starch consistently produces more butyrate than other types of dietary fiber. The production of SCFA from fibers in ruminant animals such as cattle is responsible for the butyrate content of milk and butter. Fructans are another source of prebiotic soluble dietary fibers which can be digested to produce butyrate.

Among the short-chain fatty acids, butyrate is the most potent promoter of intestinal regulatory T cells in vitro and the only one among the group that is an HCA2 ligand. It has been shown to be a critical mediator of the colonic inflammatory response. It possesses both preventive and therapeutic potential to counteract inflammation-mediated ulcerative colitis and colorectal cancer.

In 1971 while a lecturer at the University of Ghana, he won a Commonwealth Medical Fellowship Award to further his studies in surgery at the UCL Medical School. His research was in the area of Colorectal Surgery he doubled as an honorary senior lecturer in surgery at the UCL Medical School surgical unit. Furthermore, when evaluated for ERCC1, beclin-1, and beta-catenin in the stem cell region of crypts, the colonic tissues of chlorogenic acid-fed mice showed amelioration of the molecular aberrancies, suggesting that chlorogenic acid is protective at the molecular level against colon cancer. This is the first diet-related model of colon cancer that closely parallels human progression to colon cancer, both at the histopathology level as well as in its molecular profile.
Azoxymethane (AOM) is a genotoxic colonic carcinogen and is routinely used to induce colon tumours in mice. The AOM-induced tumours form in the last three centimeters of the distal colon but a p21 knock out mouse treated with AOM shows tumour distribution throughout the colon. AOM- induced tumours are characterized with mutations in the Apc gene. A novel inflammation-related mouse model of colorectal carcinogenesis combines AOM and dextran sodium sulphate (DSS) to induce colon lesions, positive for beta- catenin, COX-2 and inducible nitric oxide synthase.
Some of the less significant adverse effects of laxative abuse include dehydration (which causes tremors, weakness, fainting, blurred vision, kidney damage), low blood pressure, fast heart rate, postural dizziness and fainting; however, laxative abuse can lead to potentially fatal acid-base and electrolyte imbalances. For example, severe hypokalaemia has been associated with distal renal tubular acidosis from laxative abuse. Metabolic alkalosis is the most common acid-base imbalance observed. Other significant adverse effects include rhabdomyolysis, steatorrhoea, inflammation and ulceration of colonic mucosa, pancreatitis, kidney failure, factitious diarrhea and other problems.

Most patients with ischemic colitis recover fully, although the prognosis depends on the severity of the ischemia. Patients with pre-existing peripheral vascular disease or ischemia of the ascending (right) colon may be at increased risk for complications or death. Non-gangrenous ischemic colitis, which comprises the vast majority of cases, is associated with a mortality rate of approximately 6%. If the route was anal sex, the infected person may experience lymphadenitis and lymphangitis noted above. They may instead develop proctitis, inflammation limited to the rectum (the distal 10–12 cm) that may be associated with anorectal pain, tenesmus, and rectal discharge, or proctocolitis, inflammation of the colonic mucosa extending to 12 cm above the anus and associated with symptoms of proctitis plus diarrhea or abdominal cramps. In addition, symptoms may include inflammatory involvement of the perirectal or perianal lymphatic tissues. In females, cervicitis, perimetritis, or salpingitis may occur as well as lymphangitis and lymphadenitis in deeper nodes.

His use of rigorous aseptic technique and visits to continental surgeons like Jan Mikulicz-Radeckin at what was then the University of Breslau and Theodor Billroth in Vienna allowed him to successfully pioneer intestinal surgery in Scotland. He was one of the first to perform major gastrointestinal resections in Scotland. The operations he performed included excision of the tongue for carcinoma, closure of perforated gastric and duodenal ulcers, excision of the small bowel for tuberculous stricture, partial colectomy for colonic stricture and excision of the rectum for carcinoma, the latter procedure often performed under spinal anaesthesia.
They remain useful for identifying anatomical abnormalities when strictures of the colon are too small for a colonoscope to pass through, or in the detection of colonic fistulae (in this case contrast should be performed with iodate substances). CT and MRI scans are useful for evaluating the small bowel with enteroclysis protocols. They are also useful for looking for intra-abdominal complications of Crohn's disease, such as abscesses, small bowel obstructions, or fistulae. Magnetic resonance imaging (MRI) is another option for imaging the small bowel as well as looking for complications, though it is more expensive and less readily available.
In a developing mouse embryo, CYP26B1 is expressed in the distal tip of the forming limb bud with an abundance in the apical ectodermal ridge. In a knockout mouse model, mice manifest with severe limb malformations and die after birth due to respiratory distress. However, if the expression of CYP26B1 is conditionally deleted only prior to E9.5, the limbs are not as severely truncated and more digits are visible. Research suggests that this difference is attributable to the timing of chrondroblast differentiation. CYP26B1 has been shown to be over-expressed in colorectal cancer cells compared to normal colonic epithelium.

HSPB6 was first identified in 1994 when it was isolated from rat and human skeletal muscle as a complex with HSPB1 (also known as HSP27) and HSPB5 (also known as αB-crystallin). HSPB6 is expressed in multiple tissues; however, HSPB6 is most highly and constitutively expressed in different types of muscle including vascular, airway, colonic, bladder, uterine smooth muscle, cardiac muscle and skeletal muscle.
In a steady state, TGF-beta and AhR ligands induce low expression of IL-22 along with high expression of AhR, c-MAF, IL-10, and IL-21 that might play a protective role in cell regeneration and host microbiome homeostasis. Th17 cells mediate the regression of tumors in mice, but were also found to promote tumor formation induced by colonic inflammation in mice. Like other T helper cells, Th17 cells closely interact with B cells in response to pathogens. Th17 cells are involved in B cell recruitment through CXCL13 chemokine signaling, and Th17 activity may encourage antibody production.
Sonographic imaging and ultrasound were used to examine two cases of oesophagostomiasis in the Nalerigu hospital in northern Ghana. The technology allowed for the detection of intestinal and abdominal wall modules, as well as their size, type and location in the case of the ultrasound. Multinodular disease was representedby nodular colonic lesions and pseudokidney appearances, while the single-nodular Dapaong tumor had the appearance of an echo-free lumen surrounded by a defined but badly reflective wall. The ability to diagnose oesophagostomiasis via ultrasound can reduce the number of excessive invasive surgeries and put greater emphasis on chemotherapy.
Monomeric LPS is then transferred to MD-2 pre-complexed with TLR4 on macrophages and monocytes. This leads to release of pro-inflammatory cytokines and nitric oxide, which may lead ultimately to septic shock depending on the strength of response. Vascular endothelial cells also express TLR4 and MD-2 and so respond to LPS directly, as well as via cytokines and nitric oxide. Bronchial epithelial cells and colonic epithelial cells also express TLR4, but as they do not express MD-2 they rely on LPS precomplexed with serum MD-2 in order to signal to LPS.
These polyps are more difficult to remove endoscopically, and polypectomy in these cases has a higher risk of complication. Sessile polyps up to 10mm can often be removed by snare polypectomy. Polyps over 10mm may have to be removed piecemeal by snare polypectomy. The use of electrocautery over a large area has a significant risk of causing colonic perforation; to reduce this chance, and to facilitate the polypectomy, sterile fluid (saline or colloid, with methylene blue dye added) can be injected under the base of the polyp to raise it away from the muscular layers of the colon.
Small instestinal bacterial overgrowth (SIBO) - the patient is either given a challenge dose of glucose, also known as dextrose (75–100 grams), or lactulose (10 grams). A baseline breath sample is collected, and then additional samples are collected at 15 minute or 20 minute intervals for 2 hours. Positive diagnosis for a lactulose SIBO breath test - typically positive if the patient produces approximately 20 ppm of hydrogen and/or methane within the first 60–90 minutes (indicates bacteria in the small intestine), followed by a much larger peak (colonic response). This is also known as a biphasic pattern.
A low obstruction is suspected on plain film, but needs follow up with a gastrografin enema, which itself can be therapeutic. The differential for low obstruction is ileal atresia, meconium ileus, meconium plug syndrome and Hirschsprung disease. In cases of meconium ileus or ileal atresia, the colon distal to the obstruction is hypoplastic, usually less than 1 cm in caliber, as development of normal colonic caliber in utero is due to the passage of meconium, which does not occur in either of these conditions. When diffusely small caliber is seen, it is referred to as microcolon.
The sand can cause colic signs similar to other impactions of the large colon, and often causes abdominal distention As the sand or dirt irritates the lining of the bowel it can cause diarrhea. The weight and abrasion of the sand or dirt causes the bowel wall to become inflamed and can cause a reduction in colonic motility and, in severe cases, leads to peritonitis. Diagnosis is usually made by history, environmental conditions, auscultation of the ventral abdomen, radiographs, ultrasound, or fecal examination (See Diagnosis). Historically, medical treatment of the problem is with laxatives such as liquid paraffin or oil and psyllium husk.
These cysts on reaching the terminal ileum region of the gastrointestinal tract give rise to a mass of proliferating cells, the trophozoite form of the parasite, by the process of excystation. Symptoms of this infection include diarrhea with blood and mucus, and can alternate between constipation and remission, abdominal pain, and fever. Symptoms can progress to ameboma, fulminant colitis, toxic megacolon, colonic ulcers, leading to perforation, and abscesses in vital organs like liver, lung, and brain. Amoebiasis can be treated with the administration of anti-amoebic compounds, this often includes the use of Metronidazole, Ornidazole, Chloroquine, Secnidazole, Nitazoxanide and Tinidazole.
Sessile polyps larger than 2 cm usually contain villous features, have a higher malignant potential, and tend to recur following colonoscopic polypectomy. Although polyps do not carry significant risk of colon cancer, tubular adenomatous polyps may become cancerous when they grow larger. Larger tubular adenomatous polyps have an increased risk of malignancy when larger because then they develop more villous components and may become sessile. It is estimated that an individual whose parents have been diagnosed with an adenomatous polyp has a 50% greater chance to develop colon cancer than individuals with no family history of colonic polyps.
Screening for colonic polyps as well as preventing them has become an important part of the management of the condition. Medical societies have established guidelines for colorectal screening in order to prevent adenomatous polyps and to minimize the chances of developing colon cancer. It is believed that some changes in the diet might be helpful in preventing polyps from occurring, but there is no other way to prevent the polyps from developing into cancerous growths than detecting and removing them. Colon polyps as they grow can sometimes cause bleeding within the intestine, which can be detected by an occult blood test.
Not only do unabsorbed nutrients contribute to stool mass but mucosal fluid and electrolyte secretion is also increased in diseases associated with mucosal inflammation such as coeliac disease. In addition, unabsorbed fatty acids, converted to hydroxy-fatty acids by colonic flora, as well as unabsorbed bile acids both impair absorption and induce secretion of water and electrolytes by the colon adding to stool mass. Weight loss is common among patients with significant intestinal malabsorption but must be evaluated in the context of caloric intake. Some patients compensate for fecal wastage of unabsorbed nutrients by significantly increasing their oral intake.
The short-chain fatty acids are produced in the large intestine where they are rapidly absorbed from the colon, then are metabolized in colonic epithelial cells, liver or other tissues. The fermentation of resistant starch produces more butyrate than other types of dietary fibers. Modest amounts of gases such as carbon dioxide, methane, and hydrogen are also produced in intestinal fermentation. One review estimated that the acceptable daily intake of resistant starch may be as high as 45 grams in adults, an amount exceeding the total recommended intake for dietary fiber of 25–38 grams per day.

In cancers, loss of expression of genes occurs about 10 times more frequently by transcription silencing (caused by somatically heritable promoter hypermethylation of CpG islands) than by mutations. As Vogelstein et al. point out, in a colorectal cancer there are usually about 3 to 6 driver mutations and 33 to 66 hitchhiker or passenger mutations. In contrast, in colon tumors compared to adjacent normal-appearing colonic mucosa, there are about 600 to 800 somatically heritable heavily methylated CpG islands in promoters of genes in the tumors while these CpG islands are not methylated in the adjacent mucosa.
The structures of the most abundant (−)-epicatechin metabolites present in the systemic circulation and in urine are depicted. Catechins are metabolised upon uptake from the gastrointestinal tract, in particular the jejunum, and in the liver, resulting in so-called structurally-related epicatechin metabolites (SREM). The main metabolic pathways for SREMs are glucuronidation, sulfation and methylation of the catechol group by catechol- O-methyl transferase, with only small amounts detected in plasma. The majority of dietary catechins are however metabolised by the colonic microbiome to gamma-valerolactones and hippuric acids which undergo further biotransformation, glucuronidation, sulfation and methylation in the liver.
While the total mesocolic excision (TME) operation has become the surgical gold standard for the management of rectal cancer, this is not so for colon cancer. Recently, the surgical principles underpinning TME in rectal cancer have been extrapolated to colonic surgery. Total or complete mesocolic excision (CME), use planar surgery and extensive mesenterectomy (high tie) to minimise breach of the mesentery and maximise lymph nodes yield. Application of this T/CME reduces local five-year recurrence rates in colon cancer from 6.5% to 3.6%, while cancer-related five-year survival rates in patients resected for cure increased from 82.1% to 89.1%.
The elevated number of IgG-producing cells in the colonic mucosa of ulcerative colitis patients is largely committed to producing IgG against Tropomyosin 5-related epitopes. Tropomyosin 5 is, therefore, capable of inducing a significant T-cell response. A physicochemical analysis of common structural motifs present in 109 human autoantigens revealed that tropomyosins have the highest number of such motifs, and thus a very high propensity to act as autoantigens. In addition to the role tropomyosins play in ulcerative colitis, tropomyosin antibodies have also been reported in acute rheumatic fever and the inflammatory disorder Behcet's syndrome.
Humans with high levels of the diet-related bile acid deoxycholate (DOC) in their colons are at a substantially increased risk of developing colon cancer (see Bile acids and colon cancer). A diet-related mouse model of colon cancer was devised. In this model, wild type mice are fed a standard diet plus DOC to give a level of DOC in mouse colon comparable to that in the colons of humans on a high fat diet. After 8–10 months, 45% to 56% of the mice developed colonic adenocarcinomas, and no mice had cancers of the small intestine.
Turner-Warwick's father was a consultant surgeon who specialised in colonic surgery and in varicose veins and from an early age, Turner-Warwick decided that he was going to be a physician. He took his early education at Bedales School before matriculating to read medicine at Oriel College, Oxford in 1942. At Oxford he completed an honours degree in Natural science. Also, the effects of defects in the colonic expression of IL-22 and genes coding for antimicrobial proteins in Card9 KO mice could be reversed by FICZ. Administration of AhR agonists in different animal models of colitis has underscored the role of AhR in gut inflammation. After FICZ treatment, the severity of different types of induced colitis in mice significantly decreased, characterized by the down- regulation of pro-inflammatory cytokines and production of IL-22. Moreover, administration of anti-IL-22 prevented the anti-inflammatory effect of FICZ, demonstrating that the therapeutic effect of FICZ at least is partially mediated by IL- 22.
If the stem cells at the base of the crypt express ERCC4 (XPF), generally all several thousand cells of the crypt will also express ERCC4 (XPF). This is indicated by the brown color seen by immunostaining of ERCC4 (XPF) in almost all the cells in the crypt in panel C of the image in this section. A similar expression of PMS2 and ERCC1 occurs in the thousands of cells in each normal colonic crypt. The tissue section in the image shown here was also counterstained with hematoxylin to stain DNA in nuclei a blue- gray color.
For ERCC4 (XPF) (C), there is absent expression in most of the cell nuclei of the crypts and in the colonic lumen in this area of tissue, but detectable expression at the neck of some crypts. The reductions or absence of expression of these DNA repair genes in this tissue appears to be due to epigenetic repression. Original image, also in a publication. ERCC4 (XPF) is deficient in about 55% of colon cancers, and in about 40% of the colon crypts in the epithelium within 10 cm adjacent to the cancers (in the field defects from which the cancers likely arose).
The polyps themselves carry little malignant potential but because of potential coexisting adenomas there is a 15% chance of colonic malignancy. Juvenile polyps are hamartomatous polyps which often become evident before twenty years of age, but can also be seen in adults. They are usually solitary polyps found in the rectum which most commonly present with rectal bleeding. Juvenile polyposis syndrome is characterised by the presence of more than five polyps in the colon or rectum, or numerous juvenile polyps throughout the gastrointestinal tract, or any number of juvenile polyps in any person with a family history of juvenile polyposis.
Butyrate possesses both preventive and therapeutic potential to counteract inflammation-mediated ulcerative colitis (UC) and colorectal cancer. The reason why butyrate is an energy source for normal colonocytes and induces apoptosis in colon cancer cells, is due to the Warburg effect in cancer cells, which leads to butyrate not being properly metabolized. This phenomenon leads to the accumulation of butyrate in the nucleus, acting as a histone deacetylase (HDAC) inhibitor. One mechanism underlying butyrate function in suppression of colonic inflammation is inhibition of the IFN-γ/STAT1 signaling pathways at least partially through acting as a HDAC inhibitor.
There is also a binary toxin (AB toxin), but its role in disease is not fully understood. Additional virulence factors include an adhesin factor that mediates the binding to human colonic cells and a hyaluronidase.[Medical Micriobiology, Fifth Edition, Patrick Murray, Elsevier Mosby, 2005, page 412] The bacterium also produces the chemical para-cresol, which inhibits the growth of other microbes in its vicinity and allows it to outcompete normal human gut flora. Antibiotic treatment of C. diff infections may be difficult, due both to antibiotic resistance and physiological factors of the bacterium (spore formation, protective effects of the pseudomembrane).
Much of Wexner's work has been focused on improving surgical techniques for the avoidance of permanent stomas in patients with colorectal cancer, ulcerative colitis, and fecal incontinence. He introduced a popular modification to the technique for creating an ileo-anal pouch , or j-pouch for ulcerative colitis, in which double stapling is used in place of sutures to improve results. He also popularized the colonic j-pouch for patients with rectal cancer. The J-pouch, an alternative to a permanent ileostomy or colostomy, allows patients who have had their colons removed to continue to have regular bowel movements without an ostomy bag.

FMT involves restoration of the colonic microflora by introducing healthy bacterial flora through infusion of stool via colonoscopy, enema, orogastric tube, or by mouth in the form of a capsule containing feces from a healthy donor, which in some cases is freeze-dried. With CDI becoming more common, FMT is gaining increasing prominence, with some experts calling for it to become the first-line therapy for CDI. FMT has been used experimentally to treat other gastrointestinal diseases, including colitis, constipation, irritable bowel syndrome, and neurological conditions, such as multiple sclerosis and Parkinson's. In the United States, human feces has been regulated as an experimental drug since 2013.
Naturopaths are often opposed to mainstream medicine and take an antivaccinationist stance. The particular modalities used by a naturopath vary with training and scope of practice. These may include herbalism, homeopathy, acupuncture, nature cures, physical medicine, applied kinesiology, colonic enemas, chelation therapy, color therapy, cranial osteopathy, hair analysis, iridology, live blood analysis, ozone therapy, psychotherapy, public health measures and hygiene, reflexology, rolfing, massage therapy, and traditional Chinese medicine. Nature cures include a range of therapies based on exposure to natural elements such as sunshine, fresh air, or heat or cold, as well as nutrition advice such as following a vegetarian and whole food diet, fasting, or abstention from alcohol and sugar.
"The typical CT findings are a solitary large non-enhancing omental mass with heterogeneous attenuation, which is most often located in the right lower quadrant, deep to the rectus abdominis muscle and either anterior to the transverse colon or anteromedial to the ascending colon". Omental Infarction can be difficult to differentiate from diverticulitis however omental infarction is not normally attributed with bowel wall thickening. It is rare that the colonic wall will be thickened due to spread of the inflammation from the omentum (a fold of peritoneum connecting or supporting abdominal structures) to the tenia omentalis of the colon. Diverticulitis: Diverticulitis normally happens in older patients than in epiploic appendagitis.

A 2004 study published in Diabetes Care journal, compared Australian-born and Southern European-born Australians' risk of diabetes, using, among many factors, body mass index. A study of the Mediterranean diet, in relation to heart disease and colonic cancer, compared intragroup eating behaviours, with elderly Greek Australians living in Melbourne compared with Southern European Australians (Greeks, Italians, and others) aged 25-65. The results found that the pan-ethnic group consumed more pork, shellfish, meat in general, eggs, soft drinks and alcohol, and less chicken, organ meat, fish, stone fruit, bananas.
In this role she led people in spoken affirmations, which she believes would cure their illnesses, and became popular as a workshop leader. She also recalled how she had studied Transcendental Meditation with the Maharishi Mahesh Yogi at the Maharishi International University in Fairfield, Iowa. Hay described how in 1977 or 1978 she was diagnosed with "incurable" cervical cancer, and how she came to the conclusion that by holding on to her resentment for her childhood abuse and rape she had contributed to its onset. She reported how she had refused conventional medical treatment, and began a regime of forgiveness, coupled with therapy, nutrition, reflexology, and occasional colonic enemas.
A quicker form of the dry enema utilizes the injection of a small amount of water-based lubricant such as KY Jelly into the rectum via a non-hypodermic syringe, such as an oral syringe, or from some other source. Then again since the glycerin itself is an effective producer of the desired contraction of the colonic muscles it is simplerand more easily controlledto introduce 5–10 cc of glycerin directly into the rectum. Specialist syringes are available for this purpose but are hard to find. An alternative is to use an enema nozzle which has an intake end which is compatible with a standard hypodermic syringe.
The bioavailability of quercetin in humans is low and highly variable (0–50%), and it is rapidly cleared with an elimination half- life of 1–2 hours after ingesting quercetin foods or supplements. Following dietary ingestion, quercetin undergoes rapid and extensive metabolism that makes the biological effects presumed from in vitro studies unlikely to apply in vivo. Quercetin supplements in the aglycone form are far less bioavailable than the quercetin glycoside often found in foods, especially red onions. Ingestion with high-fat foods may increase bioavailability compared to ingestion with low-fat foods, and carbohydrate-rich foods may increase absorption of quercetin by stimulating gastrointestinal motility and colonic fermentation.

S. bovis is a human pathogen that has been implicated as a causative agent of endocarditis, urinary tract infections, and more rarely, sepsis and neonatal meningitis. S. bovis has long been associated with colorectal cancer; however, not all genospecies are associated equally. A 2011 meta-analysis on the association between S. bovis biotypes and colonic adenomas/carcinomas revealed that patients with S. bovis biotype I infection had a strongly increased risk of having colorectal cancer (pooled odds ratio: 7.26; 95% confidence interval: 3.94–13.36), compared to S. bovis biotype II- infected patients. This analysis suggests S. bovis should no longer be regarded as a single bacterial entity in clinical practice.
While transient IFN-γ signaling is generally associated with normal host immune response, chronic IFN-γ signaling is often associated with chronic inflammation. It has been shown that butyrate inhibits activity of HDAC1 that is bound to the Fas gene promoter in T cells, resulting in hyperacetylation of the Fas promoter and up-regulation of Fas receptor on the T-cell surface. Similar to other HCA2 agonists studied, butyrate also produces marked anti-inflammatory effects in a variety of tissues, including the brain, gastrointestinal tract, skin, and vascular tissue. Butyrate binding at FFAR3 induces neuropeptide Y release and promotes the functional homeostasis of colonic mucosa and the enteric immune system.
MBD4 mRNA expression is reduced in colorectal neoplasms due to methylation of the promoter region of MBD4. A majority of histologically normal fields surrounding the neoplastic growths also show reduced MBD4 mRNA expression (a field defect) compared to histologically normal tissue from individuals who never had a colonic neoplasm. This indicates that an epigenetic deficiency in MBD4 expression is a frequent early event in colorectal tumorigenesis. While other DNA repair genes, such as MGMT and MLH1, are often evaluated for epigenetic repression in many types of cancer, epigenetic deficiency of MBD4 is usually not evaluated, but might be of importance in such cancers as well.

Gynecological cancers account for about 1 in 4 cases (primarily ovarian cancer and also uterine cancer).
Dr. John Henry Tilden, 1851-1940 Tilden believed that toxemia, the build up of toxins and waste in the body, was “the universal cause of all diseases”. He believed that only a strict adherence to his dietary principles, a prudent lifestyle and proper hygiene would restore health and cure all illness. Tilden's techniques taught at the school included fasting, prescribed menus of carefully balanced food combinations, restrictions and rules on liquid intake, the necessity of fresh air and sunshine, colonic cleansing, rest and the elimination of stress both mental and physical. His philosophy of the body healing itself rather than outside interventions such as drugs or surgery foreshadowed modern naturopathy.

The biggest difference noted is the additional symptoms caused by the additional HSCR which included intestinal obstruction and colonic aganglionosis.
While many Escherichia are commensal members of the gut microbiota, certain strains of some species, most notably the serotypes of E. coli, are human pathogens, and are the most common cause of urinary tract infections, significant sources of gastrointestinal disease, ranging from simple diarrhea to dysentery-like conditions, as well as a wide range of other pathogenic states classifiable in general as colonic escherichiosis. While E. coli is responsible for the vast majority of Escherichia-related pathogenesis, other members of the genus have also been implicated in human disease. Escherichia are associated with the imbalance of microbiota of the lower reproductive tract of women. These species are associated with inflammation.
Total dietary fiber is not standard on pet food labels and generally only crude fiber is provided, which does not given an indication of the solubility of fermentability of an ingredients. If non-soluble or non-fermentable fiber is found within the diet, the animal is likely to experience increased fecal bulk and moisture as well as a diluted energy intake. Consequently, dogs may experience decreased nutrient absorption, dehydration, pathogenic bacterial growth in the gut and diarrhea. Avoiding diets with crude fiber levels over 3% or high inclusions of ingredients such as cellulose or peanut hulls will assist in preventing some consequences such as pathogenic bacterial growth, colonic atrophy and dehydration.
However, the histologic appearance varies, and evidence of chronic inflammation is seen in some cases, including intraepithelial lymphocytes or basal lymphocytes and crypt architecture distortion. Histologic inflammation may occur as early as 1-2 weeks after immune checkpoint inhibitor therapy, well before the onset of symptoms. Anti-PD-1 induced colitis may lead to more CD8+ T cell inflammation, whereas Anti-CTLA4 induced colitis may involve more CD4+ T cell infiltration and higher mucosal levels of the inflammatory molecule TNF alpha. Familial adenomatous polyposis (FAP) is an autosomal dominant inherited condition in which numerous adenomatous polyps form mainly in the epithelium of the large intestine. While these polyps start out benign, malignant transformation into colon cancer occurs when they are left untreated. Three variants are known to exist, FAP and attenuated FAP (originally called hereditary flat adenoma syndrome) are caused by APC gene defects on chromosome 5 while autosomal recessive FAP (or MUTYH-associated polyposis) is caused by defects in the MUTYH gene on chromosome 1. Of the three, FAP itself is the most severe and most common; although for all three, the resulting colonic polyps and cancers are initially confined to the colon wall.

Through one port a laparoscope is introduced (usually through a 12 mm port in the umbilicus) and another port is used for introduction of a laparoscopic instrument, such as a grasper. Riding one of Shane's tears, Ozzy reenters his body with the bead in hand and replaces the missing chromosome, reviving Frank from his momentary state of death. Having narrowly cheated death, Frank commits himself to living a healthier lifestyle, and Ozzy and Drix are declared heroes, with Ozzy having been reinstated to the force with Drix as his new partner, at which point Ozzy also begins a relationship with Leah. Frank and Shane go hiking, as he attempts to live a healthier life. Phlegmming, meanwhile, is impeached from office due to his actions having nearly killed Frank and the City of Frank with him, resulting in his opponent Tom Colonic winning the election by a landslide and himself reduced to a custodian in the bowels, where he ejects himself accidentally by triggering Frank's flatulence.

In cancers, loss of expression of genes occurs about 10 times more frequently by hypermethylation of promoter CpG islands than by mutations. The WRN gene promoter is hypermethylated in about 38% of colorectal cancers. Irinotecan inactivation of TOP1 may be synthetically lethal with deficient expression of DNA repair gene MRE11.

As for Phlegmming, Thrax's rampage resulted in a massive scandal that led up to his impeachment and removal from office, with Colonic winning the election in a landslide. Following his impeachment, Phlegmming was reduced to working as a custodian in Frank's bowels and while doing so, he accidentally presses a button that ejects himself from Frank's body via flatulence, causing his own implied demise.
