Dupuy de Lôme before her 1902 refit Dupuy de Lôme was laid down at the Brest shipyard on 4 July 1888 and she was launched on 27 October 1890. A number of her forged steel armour plates proved to be defective during tests as the metallurgical techniques to harden it were still under development, but most plates were accepted anyway. The ship was commissioned for preliminary sea trials on 1 April 1892 and one boiler tube burst on 20 June, burning 16 men. The necessary modifications to fix the problem delayed the ship's completion by almost a year.

In 2010, Toyota USA announced a Limited Service Campaign (LSC 90K) to replace the rubber portion of the oil supply hose for the VVT-i actuator on the 2GR-FE (V6) engine, which were found to be defective. In all, approximately 1.6 million vehicles manufactured prior to 2008 were affected. The defective oil supply hoses were prone to degradation and eventual rupture, causing oil to rapidly leak and resulting in permanent engine damage. In legal disputes regarding product liability, a consumer expectations test is used to determine whether the product is negligently manufactured or whether a warning on the product is defective. Under this test, the product is considered defective if a reasonable consumer would find it defective. As an example, a reasonable consumer might find exposed blades on a lawnmower, without plastic guards that could be installed for pennies, to be defective because the risk of not having the plastic guards is higher than the costs of installing those guards. Apoptosis is the highly organised process of programmed cell death in which the cell degrades the nuclear DNA and signals for phagocytosis. In people with SLE and other autoimmune disorders this process is thought to be defective, causing either an increase in cell death and/or a decrease in the rate of dead cell clearance.

He transformed into a light-green semi-humanoid being seemingly composed of bog matter and became the second Glob. It fled to the Florida Everglades, menaced Rick Jones and battled the Hulk, who mistook it for the original Glob. It was incinerated by the Man- Thing's touch after it felt fear.
Ikki Tenryou (天領 イッキ Tenryō Ikki) is a lively and easygoing boy, the main protagonist although a bit timid. At first, Ikki is unable to afford a Medabot, but after finding a medal in a river, he manages to buy an outdated model, which he names Metabee; however, the medal he found appears to be defective, as Metabee is short- tempered and sarcastic. In spite of this, a strong bond grows between them after several Robattles; though Ikki is not a full-fledged Medafighter, he gradually matures through the Robattles he engages in. He tends to challenge powerful Megafighters to prove to everyone that he's the best.
The GNAS1 gene involved in both pseudohypoparathyroidism type 1a and pseudopseudohypoparathyroidism is greatly affected by imprinting. When a father who has pseudohypoparathyroidism undergoes spermatogenesis, imprinting of the GNAS1 gene inactivates both copies of his genes: one will be Functional and the other will be defective. Tissues in the body will re- activate different copies of the GNAS1 gene selectively; the kidneys will selectively activate the (functional) maternal copy while keeping the (defective) paternally-derived gene imprinted and inactive, even in normal individuals. Since the maternally-derived GNAS1 gene is functional, renal handling of calcium and phosphate is normal, and homeostasis is maintained in pseudopseudohypoparathyroidism.
The volumes of coal transported along the canal were lower than had been predicted, hampered by the lack of a proper link from Coalisland to the collieries, but trade in flaxseed, grain, rock salt, timber, fish and coarse hardware gradually built up. Interferon- γ treatment of blastocyst-stage murine embryos leads to a loss of perlecan expression on the trophectoderm, and thus an embryonic morphology and phenotype in cell culture, which is suggestive that these interferon-γ treated blastocysts would be defective in implantation.
On January 19, 2010, the Pennsylvania Department of Transportation shut down access to the Skinners Falls - Milanville Bridge when a truss member of the bridge was found to be defective. The bridge was supposed to undergo rehabilitation of the truss members later in the year for $200,000 (2010 USD) and a larger $8 million (2015 USD) rehabilitation in 2015. As of July 2013, the bridge is in operation and open to vehicle traffic. An inspection on December 11, 2015 revealed that the bridge's suspension wires were twisted and out of line, making the bridge unsafe to carry vehicles and the 114-year-old bridge will be closed indefinitely, according to the Pennsylvania Department of Transportation.
In the manga, Tsugumo is forced to resign by shareholders when Kanzaki makes an underhanded purchasing deal with a shady jumbo jet manufacturer, assuring himself a large percentage of stock which installs him as the new YAL president. The jets turn out to be defective, causing an accidental crash which kills all aboard, thus opening Kanzaki to investigation. In the end, his wedding plans are ruined when Ryoko learns Shin's whereabouts after meeting Saki Vashtal and he is arrested for embezzlement, tax evasion, and his involvement in the plane crash. The arrest is hardly the end of Kanzaki as he returns with a number of schemes until the end of the manga series where he dies in an air duel with Shin.
Both Sultan Barquq and then Sultan al-Mu'ayyad tried, in 1397 and 1424 respectively, to replace the minaret of al-Azhar with a new one in stone, but on both occasions the construction was found to be defective and had to be pulled down. The resumption of direct patronage by those in the highest positions of government continued through to the end of Mamluk rule. Improvements and additions were made by the sultans Qaytbay and Qansuh al-Ghuri, each of whom oversaw numerous repairs and erected minarets that still stand today. It was common practice among the Mamluk sultans to build minarets, perceived as symbols of power and the most effective way of cementing one's position in the Cairo cityscape.
Many consumers were angered over this settlement, representing only a fraction of what Volkswagen had paid to United States' owners. During the COVID-19 pandemic, a number of EU countries bought medical equipment including personal protective equipment and test kits from China, a number of which were found to be defective. Due to the defective medical equipment and for China having initially failed to disclose information on the COVID-19 virus, on 21 April 2020 a cross party group of 10 MEPs wrote to the President of the European Council, Ursula von der Leyen and EU High Representative Josep Borrell requesting they take action against China. On 1 May 2020, the EC President Ursula von der Leyen backed investigation into the coronavirus origin. By 17 May 2020, a coalition of 62 countries backed a joint Australian, EU push for an independent inquiry into the COVID-19 outbreak.
Moreover, Chrysler, General Motors and Ford all had various models manufactured by Japanese companies with the seat belt buckles concerned, but sold under American names such as the Dodge Stealth and the Geo series (except Prizm) under General Motors. Ford had vehicles such as the Probe manufactured by Mazda on its MX-6 platform and the Festiva made by Kia in South Korea, but engineered by Mazda that also had the seat belts. However, unlike Chrysler and General Motors, Ford did not admit that their seat belts could be defective. Initially, some Japanese manufacturers suspected that the seat belt failures were a result of user abuse, rather than a design failure; however, the nine-month investigation by NHTSA concluded that the cause of the defect was that the buckles were made of ABS plastic.
In his book The Open Society and Its Enemies, the philosopher Karl Popper used the term "the conspiracy theory of society" to denote a conception of social phenomena that he found to be defective—namely, that social phenomena such as "war, unemployment, poverty, shortages ... [are] the result of direct design by some powerful individuals and groups." In the Netflix/DreamWorks reboot, She-Ra and the Princesses of Power, Hordak is one of the many clones of Horde Prime, who cloned himself to build a vast army to conquer everything in his path. Hordak served as Horde Prime's top general until he was revealed to be defective, and was cast out by Horde Prime as a result, sent to die on the front lines, only for a portal to open and transport him to the planet Etheria in the empty dimension of Despondos. Among the Maale of southern Ethiopia, “a small minority [of men] crossed over to feminine roles. Called ashtime, these (biological) males dressed like women, performed female tasks, cared for their own houses, and apparently had sexual relations with men,” according to Donald Donham (1990).
On 28 January 2020, the federal government publicly released its approach to COVID-19 testing. Despite this, the U.S. testing effort was slow, obscuring the extent of the outbreak. Many of the 160,000 test kits produced in February were found to be defective and were not used. Academic laboratories, hospitals and private companies were not allowed to use their own tests until 29 February, when the FDA started issuing approvals for them. Initially, there were eligibility restrictions for receiving a COVID-19 test (based on recent international travel, hospitalization for respiratory illness, or contact with another person already diagnosed with COVID-19). By 27 February, fewer than 4,000 tests had been conducted in the United States. On March5, the CDC relaxed its restrictions, allowing doctors to decide whom to test; a week later, nearly 14,000 tests had been performed. Federal funding was used to run 41 "community-based testing sites" (CBTS), but the federal government said it would stop this funding on 10 April; as a result of that decision, some of these testing sites were expected to close, shifting demand to hospitals.

The primary defect associated with iminoglycinuria is a homozygous (recessive) mutation of the SLC36A2 (PAT2) gene. One of several membrane transport proteins in the solute carrier family of amino acid transporters, PAT2 is the high-affinity renal transporter of glycine, proline and hydroxyproline found to be defective in both alleles when iminoglycinuria is present in an individual. This is in contrast to the fact that when only one PAT2 allele is defective, hyperglycinuria will be present instead of iminoglycinuria. These findings delineate iminoglycinuria as the homozygous form of hyperglycinuria, with the former having a higher degree of urinary excretion of glycine and imino acids correlating to mutations in both alleles. Another mutation suspected to convey the iminoglycinuria phenotype may be found in the SLC36A1 (PAT1) gene. Identified as the low-affinity intestinal transporter of glycine and imino acids, PAT1 works in cooperation with the renal sodium-hydrogen exchanger NHE3 (SLC9A3).
