A 2006 meta-analysis found evidence for the efficacy of stepped care: minimal intervention (e.g. send an advisory letter, or meet large number of patients to advise discontinuation), followed by systematic tapered discontinuation alone without augmentation if the first try was unsuccessful. Cognitive behavioral therapy improved discontinuation success rates for panic disorder, melatonin for insomnia, and flumazenil or sodium valproate for general long-term benzodiazepine use. A ten-year follow-up found that more than half of those who had successfully withdrawn from long-term use were still abstinent two years later, and that if they were able to maintain this state at two years, they were likely to maintain this state at the ten- year followup.

Courage International, also known as Courage Apostolate and Courage for short, is an approved apostolate of the Catholic Church that counsels "men and women with same-sex attractions in living chaste lives in fellowship, truth and love". Based on a treatment model for drug and alcohol addictions used in programs like Alcoholics Anonymous (AA), Courage runs a 12-step program aimed at helping gay people remain abstinent from sex. The organization runs support groups led by a priest to encourage its members to abstain from acting on their sexual desires and to live according to the teachings of the Catholic Church on homosexuality. Courage also has a ministry geared towards the relatives and friends of gay people called Encourage.

Drs. Linda Sobell and Mark Sobell established the Guided Self-Change (GSC) model for treating substance use disorders while working as professors at the University of Toronto, Canada in the 1980's. The GSC is a brief, motivational cognitive-behavioral harm reduction treatment designed to offer an alternative to abstinent only treatment programs such as Alcoholics Anonymous or 12 steps. By the 1970's a considerable amount of research began to appear demonstrating that alcohol problems occurred on a continuum ranging from mild to severe, mild cases of alcohol problems were more prevalent than severe cases, and alcohol problems were not necessarily progressive (i.e., problems did not always worsen without treatment or intervention).

Casparus Bernardus (Cas) Oorthuys, born 1908 in Leiden, was the fourth child and first son of Dorothea Catherina Helena Christina de Stoppelaar (1875–1948) and Reverend Gerardus Oorthuys (1876–1959), who started his career as a pastor in Brakel. In 1909 the family moved to Amsterdam, where from 1921 Oorthuys attended the Amsterdam Lyceum for three years before enrolling in the technical school in the Timorplein in Amsterdam. From 1926 to 1930 he studied architecture at the School for Architecture, Decorative Arts and Art Crafts in Haarlem and, after the closure of this school in 1927, in the architecture department of the Haarlem MTS, during which time he joined the free-thinking Dutch Association of Abstinent Students, and first took up photography.

"Abstinence plus" (also known as comprehensive sex education) covers abstinence as a positive choice, but also teaches about contraception and the avoidance of STIs when sexually active. A 2002 study conducted by the Kaiser Family Foundation found that 58% of secondary school principals describe their sex education curriculum as "abstinence plus". Abstinence-only sex education tells teenagers that they should be sexually abstinent until marriage and does not provide information about contraception. In the Kaiser study, 34% of high-school principals said their school's main message was abstinence-only. Among 48 of the 50 U.S. states (the exceptions were North Dakota and Wyoming) in their 2005 state laws and policies, 21 stressed abstinence-only sexual education and 7 emphasized that abstinence should be taught in their state's sexual education programs.

According to this paradigm, the primary force driving an addict's predicament is what Trimpey calls the "addictive voice", which can physiologically be understood as being related to the parts of the human brain that control our core survival functions such as hunger, sex, and bowel control. Consequently, when the desires of this "voice" are not satiated, the addict experiences anxiety, depression, restlessness, irritability, and anhedonia (inability to feel pleasure). In essence, the RR method is to first make a commitment to planned, permanent abstinence from the undesirable substance or behavior, and then equip oneself with the mental tools to stick to that commitment. Most important to recovering addicts is the recognition of this addictive voice, and determination to remain abstinent by constantly reminding themselves of the rational basis of their decision to quit.

Parhez was born on 21 August 1611 in Agra to Prince Khurram (the future emperor Shah Jahan) and his first wife Kandahari Begum. She was named 'Parhez Banu Begum' (Persian: "the abstinent Princess") by her paternal grandfather, Emperor Jahangir. However, in the Maasir-i-Alamgiri, she is referred to as Purhunar Banu Begum. Her father, Prince Khurram, was the third son of Emperor Jahangir, while her mother, Kandahari Begum, was a princess of the prominent Safavid dynasty of Iran (Persia) and was a daughter of Sultan Muzaffar Husain Mirza Safavi (who was a direct descendant of Shah Ismail I). Parhez was Shah Jahan's first child and his eldest daughter and was brought up by her step-great-grandmother, the dowager empress Ruqaiya Sultan Begum, who had been Emperor Akbar's first and chief wife, and who had also brought up her father, Shah Jahan.

Hélder Trindade, President of the Portuguese Institute of Blood and Transplantation (IPST), stated in 2015 that sexually abstinent homosexuals may give blood, but that MSM is definitely seen as a risk factor. In Finland, the parliamentary ombudsman launched an investigation on the possible unconstitutionality of the lifetime ban in January 2006. In June 2008, it was concluded that the ban was not unlawful in Finland as it is based on "appropriately reasoned epidemiological information" and because it is related to sexual behaviour rather than sexual orientation.

The Christian Right, who initially stood against having school-based sex education programs, began supporting abstinence-only programs due to the focus on marriage. As a driving force behind the abstinence movement, they have focused on getting more funding for these programs and have also been successful in creating more awareness surrounding abstinence through their use of activism. To the New Right, the abstinence-only sex education movement was an opportunity and an avenue to change the current view and status of sexuality in America, aligning it more with their own values, and as a result, creating individuals who remained abstinent until marriage, and thus, citizens who embodied values of self- discipline and morality. In addition, abstinence-only programs utilize ideas of individualism and personal responsibility when educating young people about sex, seeking to create individuals who take responsibility for sexuality and morality.

Results of the study funded by the Substance Abuse Policy Research Program (SAPRP) of the Robert Wood Johnson Foundation appeared in the Journal of the American Medical Association in April 2009. This first controlled assessment in the U.S. of the effectiveness of Housing First, specifically targeting chronically-homeless alcoholics, showed that the programme saved taxpayers more than $4 million over the first year of operation. During the first six months, the study reported an average cost-savings of 53 percent (even after considering the cost of administering the housing's 95 residents)—nearly $2,500 per month per person in health and social services, compared to the per month costs of a wait-list control group of 39 homeless people. Further, despite the fact residents are not required to be abstinent or in treatment for alcohol use, stable housing also results in reduced drinking among homeless alcoholics.

A 2020 systematic review of the media on pornography, published in the journal Social Forces, described "These claims do not necessarily come from scientific experts. Instead, we find that newspaper articles draw from a variety of professionals who are not scientists" and mention that "Rhodes is quoted repeatedly reflecting that he was 'addicted to internet porn' and shares the personal consequences." They conclude "journalists and political actors are overextending scientific findings to advance their media markets and political agendas"..." to codify gender stereotypes and normative heterosexuality". A 2020 paper stated that NoFap appears to have been specifically targeted by such groups, writing, "the struggle for the 'remasculinization' of white men by overcoming porn (addiction) had to be an antisemitic one: a fight against 'Jewish pornography' and 'Jewish filth,' in which other current anti-porn actors such as NoFap should join". NoFap has been described by a researcher as a "racist culture war...was already implied in the call to join the ‘movement’ by becoming abstinent".

Along these lines, the noted German philosopher Friedrich Nietzsche spoke of the positive physiological effects of abstinence: "The reabsorption of semen by the blood ... perhaps prompts the stimulus of power, the unrest of all forces towards the overcoming of resistances ... The feeling of power has so far mounted highest in abstinent priests and hermits" (quoted by Walter Kaufman in his classic, Nietzsche: Philosopher, Psychologist, Antichrist, p. 222). Before the "sexual revolution" of the 1960s, it was commonly believed by members of the medical profession that numerous mental and physical diseases in men were caused primarily by loss of nutrients through seminal discharge, and that the deliberate conservation of this substance would lead to increased health, vitality, and intellectual prowess. This also applied to masturbation, which was also thought to lead to bedwetting and hairy palms. Some advantages in favor of sexual abstinence were also claimed by Walter Siegmeister, better known as Dr. Raymond W. Bernard A.B., M.A., PhD, an early 20th-century American alternative health, esoteric writer, author and mystic, who formed part of the alternative reality subculture.
